尿NGAL在ESWL术前后变化的关联性研究

摘　　要：目的前瞻性研究尿中性粒细胞明胶酶相关性载脂蛋白(uNGAL)在早期预测ESWL术后肾损伤的变化规律及临床意义,以寻求较好的早期预测体外冲击波碎石术(ESWL)肾损伤的生化检测指标,并为2次ESWL术的间隔时间提供依据。方法筛选肾结石患者30例,分别收集其ESWL术前、术后2、6、12、24、48 h及1、2 w的尿液标本,应用酶联免疫吸附测定法(ELISA)检测患者uNGAL水平,分析其不同时段测得的NGAL数值,从而推论NGAL与ESWL术所致的早期肾损伤的肾功能变化的关联性。结果 uNGAL在ESWL术前及术后各时间点的表达有一定规律,具体表现为术后uNGAL表达较术前有明显变化,uNGAL在术后2 h开始升高,在6～24 h达到高峰,48 h后缓慢下降,1 w时仍高于术前水平,2 w时恢复到术前水平。结论 uNGAL可以作为早期肾损伤的评价指标,检测uNGAL浓度可能成为评价ESWL所致肾损伤的无创并有效的检测手段,可以指导两次ESWL的间隔时间。Objective To study urine NGAL（neutrophil gelatinase-associated lipocalin,uNGAL） test on forecast the clinical significance of early renal injury after ESWL and to look for a better biochemical criterion that early prediction of acute kidney injury（AKI）,and to provide the basis for the time interval of ESWL.Methods Screening of 30 patients with kidney stones and respectively collected the specimen of urine at ESWL preoperative and postoperative 2h,6h,12h,24h,48h,1w and 2w.Apply ELISA for testing the urine NGAL levels of patients and analysis the numerical of NGAL.And then deduced the NGAL associated with renal function changes of ESWL postoperative.Results The expression of postoperative urine NGAL compared with preoperative had obvious changes,NGAL levels began to rise at 2h after postoperative and reached a peak between the 6h and 24h after postoperative,and began to decline at 48h,1w was still higher than preoperative,recovered to preoperative levels after 2w.Conclusion NGAL can be used as evaluation index of early acute kidney injury.Detecting urine NGAL levels may be as a noninvasive and effective detection method to evaluation of early renal injury caused by ESWL,and it can guide the time interval of ESWL treatment.
