母婴皮肤接触及抚触在缓解新生儿疼痛中的效果研究

摘　　要：目的比较母婴皮肤接触和抚触干预对足跟采血所致新生儿疼痛的干预效果。方法选择阴道分娩足月新生儿90例，采用分段随机分组的方法随机分为母婴皮肤接触30例、抚触组30例及不做干预对照组30例，采用新生儿急性疼痛评分（DAN）评价足跟采血时3组新生儿疼痛差异，应用多功能监护仪动态记录穿刺前静息状态及穿刺结束时新生儿的心率（HR）和经皮血氧饱和度（SpO。）等指标变化，比较干预效果。结果足跟采血时，母婴皮肤接触组、抚触组及对照组疼痛面容持续时间分别为（69．27±14．7），（71．07±11．4），（98．10±22．5）S，啼哭持续时间分别为（34．16±9．7），（42．09±12．6），（67．30±12．5）S，DAN评分分别为（5．17±1．3），（4．89±1．6），（7．30±2．1）分，3组比较差异均有统计学意义（F分别为33．36，38．35，26．38；P〈0．01）；母婴皮肤接触组、抚触组及对照组采血过程HR变化差值分别为（-9．27±4．70），（-8．69±3．80），（-21．30±6．51）次／min，HR恢复至正常时间分别为（66．27±16．70），（71．434-15．90），（90．51．4-19．80）S，Sp02变化差值分别为（6．37±0．58），（5．88±0．34），（11．78±0．15）％，SpO2恢复至正常时间分别为（61．56±11．30），（64．09±12．90），（87．30±17．80）s，差异均有统计学意义（F分别为35．27，31．87，24．78，28．46；P〈0．01）。结论母婴皮肤接触和抚触干预均能减轻新生儿疼痛，母婴皮肤接触较抚触止哭效果更好。Objective To compare the effect of skin-to-skin contact and massage interventions on the pain caused by heel lance in the newborn. Methods A total of 90 newborns were recruited and divided into three group： the control group （n = 30） , the massage group （n = 30） and the skin-to-skin contact group （n = 30）. The newborns heart rate （HR）, oxygen saturation （ SpO2 ）, grimacing and crying time were observed between the three groups. Results The grimacing and crying time in the two intervention groups were significantly shorter than that in the control group [ skin-to-skin contact group： （69.27 ± 14.7） and （34. 16 ± 9.7）s, massage group： （71.07 ±_11.4） and （42.09 ±12.6）s, control group： （98.10 ±22.5）and（67.30 ±12.5 ） s; F = 33. 36,38.35 ; P 〈 0. 01 ], as well as the DAN score [ skin-to-skin contact group （ 5.17 ± 1.3 ）, massage group ： （4.89 ± 1.6）, control group： （ 7.30 ± 2.1 ） ; F = 26. 38, P 〈 0. 01 ） ]. The variety of HR and Sp02 [ skin-to-skin contact group： （ - 9.27 ± 4.70 ） times/min, （ 6.37 ± 0.58 ） %, massage group ： （ - 8.69 2 3.80） times/min, （5.88 20.34）% ,control group： （ -21.30 26.51 ） times/min, （ 11.78 ±0.15）% ], the convalescence time of HR and SpO2 [ skin-to-skin contact group： （66.27 2 16.70 ） and （61.56 ± 11.30） s, massage group： （71.43 ± 15.90） and（64.09 ± 12.90） s, control group： （90.51 ± 19.80） and（87.30 ± 17.80 ） s ] had significant difference between the three groups （ F = 35.27,31.87,24.78,28.46, respectively; P 〈 0. 01 ）. Corlchmions Skin-to-skin contact and massage can relieve pain during heel lance , and skin-to-skin contact can reduce the crying time better than massage.
