膀胱容量测定在宫颈癌根治术后膀胱功能锻炼中的应用效果

摘　　要：目的探讨膀胱容量测定用于宫颈癌根治术后膀胱功能锻炼的疗效。方法将80例宫颈癌根治术患者随机分为观察组和对照组各40例。术后观察组采用个体化放尿并测量膀胱容量，指导膀胱功能锻炼。对照组采用常规定时夹闭尿管2—3h放尿。观察两组拔管后第1次排尿情况、残余尿量、尿潴留发生率。结果观察组患者第1次排尿顺利为100．0％，高于对照组的77．5％，差异有统计学意义（χ2=10．14，P〈0．05）；观察组残余尿量（58±42）ml，低于对照组的（100±68）ml，差异有统计学意义（t=17．74，P〈0．05）；观察组尿潴留发生率为5．0％，低于对照组的22．5％，差异有统计学意义（χ2=5．16，P〈0．05）。结论拔管前测膀胱容量，进行膀胱锻炼，可以更直观地观察膀胱功能恢复，为选择拔管时机提供参考依据，有效预防宫颈癌术后尿潴留。Objective To study the effect of bladder capacity measure on the postoperative bladder functional exercise of patients with radical hysterectomy. Methods Totals of 80 cases with radical hysterectomy were randomly divided into observation group （n = 40） and control group （n = 40）. Observation group received individual removing catheter and measured bladder capacity during bladder functional exercise, while control group received routine method of clipping catheter and removing 2 - 3 hours. Then, the first urination, remnant urine volume, and incidence of uroschesis were observed and compared between two groups. Results The rate of successfully first urination was 100% in observation group and 77.5% in control group, and the difference was statistically significant（χ2 = 10.14, P 〈 0. 05 ）. Remnant urine volume in observation group was significantly lower than that in control group [ （ 58 ± 42 ） vs （ 100 ± 68 ） ml ; t = 17. 74 ; P 〈 0. 05 ]. And the incidence of uroschesis in observation group was significantly lower than that in control group（ 5.0% vs22.5%, χ2 = 5.16; P 〈 0. 05）. Conclusions Application of bladder capacity measure in the postoperative bladder functional exercise of patients with radical hysterectomy can directly observe the foundation recover of bladder, and provide reference for catheter drawing, and effectively prevent the incidence of urosehesis.
