摘  要:目的探讨彩色多普勒超声对不全纵隔子宫妊娠的诊断价值。方法选择早孕期在孝感市中心医院接受超声检查并拟诊为不全纵隔子宫妊娠45例,跟踪随访至妊娠晚期,观察纵隔子宫在妊娠不同时期声像图特点,最后经手术或产后超声证实。结果除2例在早孕期复查后确诊为子宫角部妊娠,其余43例妊娠早期表现为内膜呈Y字形,一侧宫腔内膜增厚呈蜕膜样变,一侧宫腔见偏心性孕囊回声。妊娠中、晚期:羊水内见带状回声,其基底部宽厚,与宫底或宫腔上部相连,不影响胎儿活动。38例经剖宫产手术证实,5例正常分娩产妇于产后复查子宫超声得以证实。结论彩色多普勒超声对不全纵隔子宫妊娠期声像图表现具有一定特异性,能准确诊断并指导临床。Objective To investigate the value of color Doppler ultrasound in diagnosis of pregnancy in a woman with partially septate uterus.Methods Forty five early pregnancy women proposed diagnosis of partially septate uterus by ultrasound were follow up to the late trimester of pregnancy.The ultrasonographic characteristics were observed at different trimester of pregnancy.The results were confirmed by surgery or ultrasound examination of the postpartum uterus.Results Two cases were diagnosed as angular intrauterine pregnancy by the re-examination at early trimester of pregnancy.Other 43 cases reveal Y-shape endometrium at early trimester of pregnancy.One side was with thickening endometrium like deciduosis,another side with eccentricity of the gestational sac.During middle and late pregnancy,a band echo signal could be seen in amniotic fluid,with a broad fundus connected to the fundus uteri or top of the uterine cavity.It did not influence the movement of fetus.38 cases were confirmed to be the partial septate uterus by cesarean surgery,5 cases by ultrasound examination of the postpartum uterus.Conclusion Color Doppler ultrasound examination in diagnosis of pregnancy in a woman with partially septate uterus has a certain specificity and accuracy,and can be used as clinical guiding.
