35例肾细胞癌超声诊断分析

摘　　要：目的探讨肾细胞癌（RCC）的超声表现及特征，评价超声对其的诊断价值。方法对2006年10月-2012年6月间35例肾细胞癌超声声像图总结。所有病例中瘤体小于3cm8例，瘤体3—5cm18例，瘤体大于5cm9例；CDFI血流呈抱球型12例，较丰富8例，星点状血流10例，少血流显示5例。肾透明细胞癌26例，肾颗粒细胞癌1例，肾乳头状细胞癌5例，肾肉瘤样癌1例，肾囊性肾细胞癌2例；肿瘤呈中低回声23例，无回声1例，高回声5例，混杂回声6例。分析研究肿瘤大小、有无血流、病理类型及超声声像图表现之间的联系。结果肾细胞癌的超声声像图表现多为边界较清的均匀低回声及等回声团块，肿块周边及内部血流信号多丰富，高速低阻血流信号是诊断恶性肿瘤的标记。结论超声对肾脏占住病变的诊断和良恶性病变的鉴别诊断有较高价值。Objective To study the uhrasonographic findings and characteristics of renal cell carcinoma（ RCC）, and evaluate the diagnosis value of ultrasound in RCC. Methods The clinical data of 35 patients with RCC in our hospital between October 2006 and June 2012 were analyzed retrospectively. 8 cases were with the diameter of tumors less than 3 cm, 18 cases with the di- ameter between 3 and 5 cm, and 9 patients with the diameter greater than 5 cm. 12 cases were with globular expression of blood flow in color Doppler flow imaging（CDFI） ,8 cases with abundant blood flow, 10 cases with star dot blood flow,5 cases with few blood flow. There were 26 cases of renal clear cell carcinoma, 1 case of renal granular cell carcinoma, 5 cases of renal papillary cell carcinoma, 1 case of renal sarcomatoid carcinoma,2 cases of cystic renal cell carcinoma. 23 cases were with hypoechoic mass in Color-Doppler,1 case without echo,5 cases with hypereehoic and 6 cases with mixed echo. The contact among the size of tumors, blood flow, pathological type and uhrasonographic findings were analyzed. Results On most sonograms of RCC, tumors were with clear boundaries and relatively homogeneous echoes, Echogenicity might be hypoechoic or medium echoic. Color- Doppler signals of blood flow of interior and peripheral of tumors were abundant, and high-speed resistance flow signal was the di- agnostic marker of malignant tumors. Conclusion Ultrasonographic has high value in diagnosis of reral occuping lesions and differential diagnosis of benign and malignant lesions.
