摘 要:目的比较儿童股骨干骨折的外固定架治疗及切开复位LC-DCP钢板内固定两种术式的疗效。方法 2007年4月—2010年5月期间昌黎县人民医院收治5~13岁,41例儿童股骨干骨折病例,其中通过外固定架固定20例,切开复位钢板内固定21例:所选取的病例均为股骨干骨折,并应用HSS评分系统(主观评分系统)进行结果分析评价,同时观察两种手术方法患者的手术时间、住院时间、住院费用、恢复时间及有无感染的差别。结果 21例钢板内固定患者手术后18周内的治愈率为95.6%,术后伤口全部一期愈合,无手术并发症出现;外固定架治疗的21例患者治愈率为71.4%,有4例针孔感染,骨折骨性愈合,骨折延迟愈合1例。外固定架治疗20例,6例固定失败进行第2次手术。15例术后14周内骨性愈合,4例针孔感染。结论对于儿童股骨干骨折采用切开复位LC-DCP钢板内固定较外固定架固定效果安全可靠。[ Objective ] To compare the curative effect between two surgical methods of external fixator and open reduction of LC-DCP plate internal fixation in the treatment of femoral shaft fracture in children. [ Methods ] 41 patients of femoral shaft fracture, who were aged 5-13 years old and treated in Changli People's Hospital from April 2007 to May 2010, were collected, while 20 cases were given external fixator treatment, and 21 cases were treated with open reduction of LC-DCP plate internal fixation. The results were evaluated by HSS system ( Hospital for Special Surgery). Meanwhile, the differences in operation time, hospital stay, hospi- talization expenses, recovery time and infection rate between two groups were observed. [ Results] The cure rate of 21 patients who received plate internal fixation within 18 weeks after operation was 95.6%, all of postoperative wounds? healed? by the first inten- tion, and there were no operation complications. The cure rate of 2t patients who received external fixator treatment was 71.4%, while there were 4 cases of pinhole infection, as well as 1 case of delayed union. Among 20 cases of external fixator treatment, 6 cases were failed and received the operation again, bony union were observed in 15 cases within 14 weeks, and there were 4 cases of pinhole infection. [ Conclusion]The open reduction of LC-DCP plate internal fixation has more safe and reliable effect as com- pared with external fixator in the treatment of femoral shaft fracture in children.
