普瑞巴林联合神经阻滞治疗40例老年带状疱疹后三叉神经痛临床疗效分析

摘　　要：目的观察普瑞巴林联合神经阻滞治疗老年带状疱疹后三叉神经痛的临床效果。方法选择60岁以上老年带状疱疹后三叉神经痛患者40例,采用口服普瑞巴林胶囊(150-300 mg/d)、三叉神经分支阻滞和皮损区皮内臭氧注射治疗4周,并用视觉模拟评分(VAS)和睡眠质量评分(QS)评价临床疗效。结果治疗后第1-4周与治疗前比较,VAS和QS均显著下降,差异有统计学意义(P<0.01)。40例患者治疗4周后显效31例(77.5%),好转9例(22.5%),总有效率为100.0%。服用普瑞巴林后出现嗜睡3例(7.5%),轻度头晕7例(17.5%)。结论普瑞巴林联合神经阻滞治疗老年带状疱疹后三叉神经痛效果好,不良反应较轻,操作简单、安全,是临床上较为理想的治疗选择。Objective To observe the clinical efficacy of pregabalin combined with nerve block in the treatment of trigeminal neuralgia following herpes zoster in elderly patients.Methods 40 patients aged more than 60 years old with trigeminal neuralgia following herpes zoster were selected and treated with oral pregabalin 150-300 mg/d combined with the block of the trigeminal nerve and the ozone injection therapy in the skin lesion areas for 4 weeks.The clinical effect was evaluated before and after treatment by the visual analogue scale（VAS） and the quality of sleep（QS）.Results The VAS scores were significantly decreased from（7.89±0.73） points before treatment to（1.92±0.53） points after 1-4 weeks treatment,and the QS scores were significantly reduced from（3.28±0.34） points before treatment to（0.87±0.28） points after 1-4 weeks treatment,the difference showing statistical significance（P0.01）.Among 40 cases treated for 4 weeks,31 cases were significantly effective（77.5%） and 9 cases were improved（22.5%） with the total effective rate of 100.0%.After orally taking pregabalin,3 cases（7.5%） of drowsiness and 7 cases（17.5%） of mild dizziness occurred.Conclusion Oral pregabalin combined with trigeminal nerve block is effective and safe in treating elderly trigeminal neuralgia following herpes zoster,the adverse reactions are mild,the operating is simple and safe,which is a more ideal treatment option in clinic.
