依那西普治疗活动性类风湿关节炎的临床疗效观察

摘　　要：目的探讨依那西普治疗活动性类风湿关节炎(RA)的效果及安全性。方法选取本院2010年1月～2012年12月收治的65例RA患者,根据入院的先后顺序将其随机分为观察组33例与对照组32例,对照组每周口服10～15mg甲氨蝶呤(MTX),观察组在对照组的基础上联合应用依那西普(注射用重组人Ⅱ型肿瘤坏死因子受体-抗体融合蛋白,rhTNF-Fc),每次皮下注射25mg,每周2次,治疗12周为1个疗程,根据美国风湿病学会(ACR)制订的标准进行疗效评定。结果治疗6、12周后观察组ACR20%改善标准、ACR50%改善标准均优于对照组,差异有统计学意义(P<0.05);观察组休息痛、关节压痛数、晨僵持续时间、关节肿胀数、DAS28等指标均优于治疗前及对照组治疗后,差异有统计学意义(P<0.05);两组均未出现严重的不良反应。结论依那西普治疗活动性RA效果肯定,安全性高,是较好的临床治疗方案,值得在临床上推广使用。Objective To investigate the effectiveness and safety of etanercept for patients with active rheumatoid arthritis （RA）.Methods The clinical data of 65 patients with active rheumatoid arthritis admitted to our hospital from January 2010 to December 2012 were retrospectively analyzed,and were randomly assigned into observation group （33 cases） and control group （32 cases）,according to the order of admission.The control group were treated by weekly oral 10-15 mg methotrexate （MTX）,while the observation group were given etanercept （injection of recombinant receptor of human type Ⅱ tumor necrosis factor-antibody fusion protein,rhTNF-Fc） on the base of control group,subcutaneous injection of 25 mg per time,twice a week,for a treatment course of 12 weeks.The therapeutic effect was evaluated according to the standards set by the American College of Rheumatology （ACR）.Results After treatment of 6 weeks and 12 weeks,the ACR20% and ACR50% improvement standard of observation group was respectively better than those of control group, with statistically significant difference （P＆lt;0.05）.The indes such as rest pain,the count of joint tenderness, duration of morning stiffness,and DAS28 of observation group was respectively better than those of observation group before treatment and of control group after treatment, with statistically significant difference （ P＆lt;0.05）.There were no serious adverse reactions in both groups.Conclusion For active rheumatoid arthritis,etanercept has definitely therapeutic effect and high safety,is the better clinical treatment scheme,and is worth widely clinical applying.
