摘 要:目的探讨胰岛素强化血糖控制对初发2型糖尿病(T2DM)患者β细胞功能的影响。方法选择90例初发T2DM患者随机分为观察组与对照组,两组患者均给予饮食控制与运动疗法,观察组给予胰岛素强化治疗,对照组给予口服降糖药物治疗,对比两组患者治疗前及治疗后3个月时空腹血糖(FBG)、糖化血红蛋白(HbAlc)、空腹胰岛素(FINS)、β细胞功能指数(HOMA-β)、胰岛素分泌指数(△I30/△G30)变化。结果治疗前两组患者FBG、HbAlc、FINS、HOMA-β、△I30/△G30差异无统计学意义(P均>0.05)。治疗后两组患者的FBG、HbAlc、FINS、HOMA-β、△I30/△G30较治疗前有显著改善(P均<0.05);治疗后两组患者的FBG与HbA1c比较,差异无统计学意义(P>0.05),观察组FINS、HOMA-β、△I30/△G30高于对照组(P均<0.05)。结论胰岛素强化治疗可在一定程度上延缓初发T2DM患者胰岛β细胞功能的减退,有效控制血糖,延缓病情发展。Objective To investigate the effect intensive insulin therap to βcell fuction on patients with incipient type 2 diabetes mellitus(2DM).Methods 90 cases of patients with incipient 2DM were randomized into observation groups and con trol groups,all patients taken alimentary control and exercise therapy,Patients in observation groups taken intensive insulin ther ap,and patients in control groups taken hpoglycemic drugs,FBG,HbAlc,FINS,HOMA-β and △I30/△G30 were contrasted be tween observation groups and control groups before therapia and in 3 months after therapia.Results FBG,HbAlc,FINS,HOMA-β and △I30/△G30 were indentical between observation groups and control groups before therapia(P0.05)FBG,HbAlc,FINS,HOMA-β and △I30/△G30 were improved after therapia than before therapia(P0.01);FBG and HbAlc were identical af ter therapia between observation groups and control groups(P0.05),FINS,HOMA-β and △I30/△G30were higher in observation groups than control groups after therapia(P0.01).Conclusions Intensive insulin therap could delay hypofunction of β cell in pancreatic islet on incipient 2DM,and had a important effect on controlling blood glucose and postpone progression.
