胰腺纤维钙化性糖尿病1例报告并文献复习

摘　　要：目的探讨胰腺纤维钙化性糖尿病(Fibrocalculous pancreatic diabetes,FCPD)的临床特点,提高对FCPD的认识。方法报道1例典型FCPD的诊治经过和随访结果。结果患者男性,43岁,BMI:14.2kg/m2,以反复腹痛、频繁腹泻、血糖升高起病,症状进行性加重;实验室检查:入院随机血糖38.76mmol/L,糖化血红蛋白16.3%,空腹胰岛素0.496mU/L、餐后2小时胰岛素1.30mU/L;腹部CT:胰腺实质内可见弥漫的点状和结节状钙化。结论临床上对于体型消瘦、胰岛功能差但无酮症倾向、脂肪吸收障碍的成年糖尿病患者应注意筛查FCPD;胰腺CT能清楚的显示胰腺的形态以及钙化程度,可作为常规筛查手段。Objective To discuss the features of fibrocalculous pancreatic diabetes（FCPD）,and improved the diagnosis and treatment.Methods Report a typical case of FCPD.Result A middle aged man was onset with chronic abdominal pain,diarrhea,diabetes,lab tests showed internal and external secretion of pancreas was damaged significantly,imaging tests demonstrated diffused calculus and fibrosis of the whole pancreas.Conclusion We should pay more attention on screening FCPD from adult diabetes,who with thinned body type,dysfunction of insuloma,and without ketosis trend and malabsorption of fat.Pancreas CT could described morphology and calcification degree of pancreas clearly,so it was best to use for screening.
