多层螺旋CT对十二指肠乳头旁憩室的分型诊断及其临床意义

摘　　要：目的评价CT对十二指肠乳头旁憩室（juxtapapillarydiverticulumofduodenum，JDD）的诊断价值。方法回顾性分析63例JDD（共67个）的临床表现和CT征象，将其分型并比较各型乳头旁憩室综合征（Lemmel’ss yndrome）的发生率。结果在63例中，共发现67个JDD病灶，其中47个JDD并发乳头旁憩室综合征。包括胆囊病变37例，胆总管病变30例，胰腺炎4例，还有3例并发十二指肠憩室炎。67个JDD中，Ⅰ型32个，Ⅱ型17个，Ⅲ型14个，Ⅳ型4个。各型并发乳头旁憩室综合征的发生率存在统计学差异，I型发生率最高（x2=9．132，P=0．028）。结论多层螺旋CT是诊断JDD非常实用而有效的影像学方法，可显示其与十二指肠乳头的关系，对l商床胆胰疾病的病因诊断提供有益的帮助。Abstract： Objective To evaluate the value of multi-slice spiral CT in the diagnosis of juxtapapillary duodenal diverticulum （JDD）. Methods Clinical appearances and CT images of 63 patients with JDD （n 67） were analysed retrospectively. They were di- vided into 4 types and the incidence of peripapillary diverticulum syndrome was compared. Results In 63 cases, 67 JDD were found. In 67 JDD, 47 Lemmel＇s syndrome were found, including 37 gallbladder disease, 30 dilated common bile duct diseases and 4 pancreatitis. 3 duodenal diverticulitis were found. JDD were divided into 4 types： type Ⅰ （n=32） , typeⅡ{n= 17） , typeⅢ （n=14）, and typeⅣ（n= 4） . There were statistical difference for the incidence of Lemmel＇ s syndrome among 4 types of JDD（ Ze = 9. 132, P= 0. 028）. The incidence of Lemmel＇s syndrome for type Ⅰ was the highest. Conclusion Multi-slice spiral CT can not only provide sufficient diagnostic information of JDD, but also can display the relationship between JDD and papillary, which is beneficial to deter mine the causes of clinical pancreaticobiliary disease.
