摘 要:目的探讨磁共振全身类PET成像在恶性肿瘤诊断及疗效评价中的应用价值。方法对31例确诊为恶性肿瘤的患者,应用磁共振全身弥散加权成像(WB-DWI)技术扫描,扫描数据经3D-MIP重建,黑白反转,形成类PET影像。16例与常规MRI/CT图像对比,15例与PET图像对比。比较12例患者病变区域放化疗前后的体积及ADC值变化。结果恶性肿瘤的原发灶、术后复发灶及转移灶的实性成分在DWI图像上均呈高信号,囊变及坏死部分呈以低信号为主的混杂信号。与常规MRI/CT比较的16例中,类PET成像的敏感度和特异度分别为90.7%、90.0%;常规MRI/CT的敏感度和特异度分另q为88,1%、90.3%。与PET比较的15例中,类PET成像的敏感度和特异度分别为91.9%、96.0%;PET成像的敏感度和特异度分别为86.5%、92.0%。类PET成像显示骨转移瘤和膀胱肿瘤的数目多于PET成像。12例行放化疗的患者中,7例病灶范围缩小,10例病变区域的ADC值升高。结论磁共振全身类PET成像对恶性肿瘤的筛查诊断和疗效评价具有较高的应用价值。Objective To study the application value of MRI whole-body PET-like imaging on the diagnosis and curative effect evaluation of malignant tumor. Methods Whole-body diffusion weighted imaging (WB-DWI)were performed in 31 patients with malignant tumor . The scanning data were reconstructed with 3D-MIP and black-and-white reversal in order to obtain PET-like imaging. The WB-DWI imaging of 16 cases were compared with conventional MRI/CT and 15 cases with PET imaging. The size and ADC value of lesions before and after treatment were measured and compared in 12 patients who underwent radiotherapy and chemotherapy. Results All solid ingredients of primary , relapse and metastasis tumor showed obvious high signal in DWI imaging. The cystic and necro- sis parts of lesion showed mixed signal with low-signal based. Among the 16 cases compared with conventional MRI/CT, the sensi- tivity and specificity of WB-DWI were 90. 7%,90.0%, that of conventional MRI/CT were 88. 1%,90.3%. Among the 15 cases compared with PET, the sensitivity and specificity of WB-DWI were 91.9% ,96.0%, that of PET were 86.5% ,92.0%. WB-DWI imaging displayed more skeletal metastasis and bladder tumours than PET imaging. Among 12 patients who experienced trement, the lesions became lessened in 7 cases, ADC values were rised in 10 cases. Conclusion MRI whole-body PET-like imaging has im- portant application value on screening diagnosis and curative effect evaluation of malignant tumors.
