摘 要:目的分析高场强磁共振灌注成像技术在胰腺癌中的诊断价值。方法对33例怀疑胰腺癌的患者依次进行常规MR平扫、MR灌注成像及MR增强扫描,描绘瘤体区、瘤体周边区及正常区的时间-信号曲线(TIC),并分析各区域各项灌注参数的差异性,包括斜率(Slop)、峰值(PE)、达峰时间(TTP)及信号增强率(SR)。结果胰腺癌瘤体区TIC呈缓慢上升趋势,灌注参数Slop、PE、SR小于瘤体周边区及正常区,而TTP明显延长;瘤体周边及正常区TIC早期均呈快速上升趋势,达峰值后走行略有差异,但是瘤体周边的Slop、PE明显低于正常区,TTP略延长,SR无明显差异性;Ⅰ期、Ⅱ期和Ⅲ期、Ⅳ期之间Slop、TTP、SR无明显差异性。结论 MR灌注成像是功能性成像技术,作为常规形态影像学检查的一种补充,可以直观显示胰腺癌和周边胰腺组织的血流动力学改变,间接评估瘤体内肿瘤血管的情况,为胰腺癌的准确诊断及综合治疗提供有价值的信息。Objective To analyze the diagnostic value of high field magnetic resonance perfusion imaging in pancreatic cancer. Methods 33 cases of suspected pancreatic cancer patients underwent routine MR plain, MR peffusion imaging and enhanced MR scan. The time signal curve (TIC) of the tumor zone, tumor peripheral zone and normal zone were de- picted, the perfusion parameters of various regions, including the slope (Slop) , peak enhancement ( PE ), time to peak (TrP) and signal enhancement ratio (SR) were analyzed. Results TIC of pancreatic tumor area showed a slow upward trend, Slop, PE and SR of tumor zone were less than that of the tumor peripheral area and normal areas, while the TFP was significantly prolonged. TIC of tumor periphery and normal zone showed a rapid upward trend in the early and was slightly different after reaching peak, but the Slop and PE of the tumor surrounding area were significantly lower than that of normal region, TIP extended slightly, SR had no significant differences. Slop, TYP and SR Between stage I , ]] and stage m, 1v were no significant differences. Conclusion MRI Perfusion is a functional imaging technique and can be used as a sup- plement to conventional morphological imaging modalities, which can visually display hemodynamic changes between pan- creatic cancer and the surrounding pancreatic tissue and indirectly assess the tumor blood vessels of tumors in vivo, provide valuable information of diagnosis and treatment for pancreatic cancer.
