摘　　要：目的探讨婴幼儿磁共振检查镇静方法。方法回顾性分析2008年4月至2009年5月550例年龄为1天～3岁患儿磁共振检查160例镇静失败原因。镇静剂应用方法:3个月以内患儿首次镇静选择静脉注射苯巴比妥10 mg/kg/次;3个月～3岁患儿首选10%水合氯醛,按30～40 mg/kg/次口服或灌肠。改进方法后对2009年6月至2011年12月1619例年龄为1天～3岁患儿成功进行磁共振检查1599例进行总结。镇静剂应用方法:6个月以内患儿首次镇静选择静脉注射苯巴比妥15～20 mg/kg/次;6个月～3岁患儿首选10%水合氯醛按50 mg/kg/次口服。结果在550例中,一次检查成功患儿390例(70.9%),首次未成功者160例(29.1%)。分析失败原因,与首次镇静剂用量不足、给药方法及给药时机、个体差异等因素有关。改进方法后在1619例中,检查成功患儿1599例(98.8%),未成功者20例(1.2%)。差异有统计学意义(χ2=414.186,P<0.005)。结论镇静剂首次应用剂量、正确的给药途径和用药时机的选择是保证婴幼儿磁共振检查成功的关键,应引起磁共振从业人员和儿科医师的高度重视。Objective To discuss the methods of sedation in infant with MR examination. Methods A retrospective analysis of the reason for sedation failure in 160 of 550 cases of age from 1 day to 3 yeas old children with MRI from April 2008 to May 2009. Sedative methods ： children less than 3 months for the first time sedation used intravenous phenobarbital 10 mg/kg/. Children, 3 months to 3 years old, prefer the 10% chloral hydrate, 30 -40mg/kg/oral or enema. After the improvement, a total of 1619 cases of age from 1day to 3 yeas old children had MRI from June 2009 to December 2011. 1599 of 1619 successful cases were summarized. Sedative methods ： children less than 6 months for the first time sedation used intravenous phenobarbital 15 -20 mg/kg/. Children, 6 months to 3 years, prefer 10% chloral hydrate, 50 mg/kg/o- ral. Results In 550 cases, 390 cases were one time success （70.9%）, 160 cases were first time failure （29.1% ）. The reasons for failure were inadequate sedative dosage for the first time, delivery method and timing, individual differences etc. After the improvement, in 1619 cases, 1599 cases were successful （98.8%） , 20 cases were failed （1.2%）. The difference was statistically significant （X2 = 414. 186, difference was very significant）. Conclusion The sedatives dose for the first time, the correct way and timing for medication, are the key to the success in infant with MR examination. All the radiographers and pediatricians should highly pay attention to it.
