摘  要:目的回顾性分析封闭式玻璃化冷冻技术与程序化冷冻技术在胚胎冷冻解冻及移植临床效果的差异。方法首先比较2010年11~12月采用三种开放式载杆和一种封闭式载杆对卵裂期胚胎进行的玻璃化冷冻。然后对2010年11月至2011年11月采用封闭式载杆行玻璃化冷冻和2009年1月至2010年12月采用程序化冷冻的数据进行对比,比较两组的复苏率、着床率和临床妊娠率等。结果三种开放式载杆与封闭式载杆的复苏率及着床率、临床妊娠率无显著性差异(P>0.05)。封闭式玻璃化冷冻和程序化冷冻两种冷冻方法比较显示:卵裂期胚胎采用玻璃化冷冻共2,61 9周期,解冻6,627枚胚胎,复苏6,276枚,复苏率为94.7%(6,276/6,627),1,522枚胚胎着床,着床率为24.3%(1,522/6,276),共1,1 35周期获得临床妊娠,移植妊娠率为43.3%(1,1 35/2,619),均显著高于程序冷冻组(P<0.05),后者的复苏率、着床率和移植妊娠率分别为59.9%(7,797/1 3,01 6),20.6%(1,603/7,797)和39.3%(1,224/3,11 7)。玻璃化冷冻囊胚共331周期,解冻677枚胚胎,复苏589枚,复苏率为87.0%(589/677),21 4枚胚胎着床,着床率为36.3%(214/589),168周期获得临床妊娠,移植妊娠率为50.8%(1 68/331),均显著高于程序冷冻组(P<0.05),后者共114周期,解冻21 7枚胚胎,140枚复苏,复苏率为64.5%(140/217),着床率为22.1%(31/140),妊娠率23.7%(27/114)。结论采用封闭式载杆玻璃化冷冻胚胎是一种较程序化冷冻更有效、更安全的胚胎冷冻方法。Objective: To retrospectively analyze the clinical outcome between vitrification of cleavage stage embryos with enclosed carrier and automatic program slow freezing. Methods: The clinical outcomes of three kinds of open carriers and one kind of enclosed carrier for vitrification of cleavage stage embryos from Nov. to Dec. 2010 were compared. And then the data of vitrification of embryo with enclosed carrier from Nov. 2010 to Nov. 2011 were compared with those of automatic program slow freezing from Jan. 2009 to Dec. 2010. The survival rate, implantation rate and clinical pregnancy rate of these two groups were the indexes to be compared. Results: There were no significant differences in survival rate,implantation rate and clinical pregnancy rate among three kinds of open carriers and the enclosed carrier for vitrification of cleavage stage embryos (P〈0.05). The survival rate, implantation rate and clinical pregnancy rate in vitrified cleavage stageembryos with enclosed carrier were 94.7%(6,276/6,627),24.3%(1,522/6,276),43.3%(1,135/2,619) respectively, which were significantly higher than those of slow frozen embryos ( 59.9 % (7,797/13,016), 20.6%(1,603/7,797) ,39.3% (1,224/3,117) respectively) (P^0.05). The survival rate, implantation rate and clinical pregnancy rate of vitrified blastocysts were 87.0% (589/677), 36.3% (214/589), 50.8% (168/ 331)respectively,which were significantly higher than those of slow frozen blastocyst ( 64.5 % (140/217), 22.1%(31/140) ,23.7% (27/114)respectively) (P〈0. 05) . Conclusions. Vitrification with enclosed carrier is more effective and safer than automatic program slow freezing method.
