2型糖尿病视网膜病变与血清中TG／HDL比值关系研究

摘　　要：目的探讨血清中甘油三脂与高密度脂蛋白（TG／HDL）比值与糖尿病视网膜病变之间的关系及临床意义。方法临床病例对照研究。对2011年11月至2012年5月在郑州大学第一附属医院眼科就诊的131例内分泌科住院的2型糖尿病病人按眼底照相所见分为三组，其中非糖尿病视网膜病变组（NDR）28例，非增值型糖尿病视网膜病变（NPDR）组85例，增值性糖尿病视网膜病变（PDR）18例。检测并评估总胆固醇（CHOL）、甘油三脂（TG）、高密度脂蛋白（HDL）、低密度脂蛋白（LDL）及载脂蛋白A1（APOPA1）、载脂蛋白B100（APOB100）及TG／HDL值；及其他可能影响糖尿病视网膜病变进展的相关临床指标。结果NDR组、NPDR组和PDR的TG／HDL值分别为（2．79±1．11）、（2．85±0．83）和（4．00±1．72），三组之间TG／HDL比值的差异有统计学意义（P〈0．001），Logis-tic回归分析显示TG／HDL比值与糖尿病视网膜病变程度呈明显正相关（B=1．795，P=0．032）。结论TG／HDL比值可作为评估糖尿病视网膜病变的一个有效指标，有助于糖尿病视网膜病变的早期诊断和治疗。Objective To investigate the relationship between serum triglyceride/high-density lipo- protein cholesterol （TG/HDL） ratio with Type 2 diabetic retinopathy and its clinical significance. Methods One hundred and thirty-one patients with Type 2 diabetes mellitus were divided into three groups according to their fundus condition, 28 patients with no diabetic retinopathy （NDR）, 85 with non-proliferative diabetic retinopathy （NPDR）, 18 with proliferative diabetic retinopathy （PDR）. Clini- cal parameters related to the progression of DR, including cholesterol triglycerides （TG）, high-density lipoprotein cholesterol （HDL）, LDL, and apolipoproteinAI （apoAI）, apolipoproteinB （apoB）, TG/HDL were determined and assessed. Results TG/HDL ratio was 2.79±1.11, 2.85±0.83 and 4.00±1.72 in NDR, NPDR and PDR group respectively. Difference of TG/HDL ratio among the three groups were statistical significance （P 〈0.001）. Logistic regression analysis showed a positive correlation displayed between TG/HDL ratio and DR （B =1.795, P =0.032）. Conclusions TG/HDL ratio may be used as an effective indicator for the assessment and treatment of DR.
