桂枝加厚朴杏子汤合玉屏风散联合西药治疗慢阻肺缓解期随机平行对照研究  

Randomized Controlled Study on COPD Remission Stage with the Treatment of the Guizhijiahoupoxingzi and Yupingfeng Decoction

摘　　要：[目的]观察桂枝加厚朴杏子汤合玉屏风散联合西药治疗慢阻肺缓解期疗效。[方法]使用随机平行对照方法,将50例住院及门诊患者按抽签方法随机分为两组,对照组25例舒利迭干粉剂,1吸/次,1～2次/d,吸入后尽量屏气10～15s,缓慢呼气后清水漱口,以防止口腔霉菌感染或溃疡,治疗组25例桂枝加厚朴杏子汤合玉屏风散(桂枝10g,白芍10g,川朴12g,杏仁12g,甘草6g,黄芪20g,防风12g,白术12g,生姜6g,大枣6g),按1:5的容积比用水浸泡30min,煎煮40min,煎煮2次,滤取2次煎液约200mL,早晚各1次温服,1剂/d,西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组痊愈7例,有效16例,无效2例,总有效率92.00%。对照组痊愈2例,有效15例,无效8例,总有效率68.00%。治疗组疗效优于对照组(P<0.01)。中医证候评分两组均有明显降低(P<0.01),治疗组降低优于对照组(P<0.01)。[结论]桂枝加厚朴杏子汤合玉屏风散联合西药治疗慢阻肺缓解期疗效满意,值得临床推广应用。[Objective]To observe the clinical effect of COPD remission stage treating with Guizhijiahoupoxingzi and Yupingfeng decoction.[Method]50 patients meet the diagnosis of COPD remission stage patients were randomly divided into two groups by lot,25 patients were randomly divided into treatment group and control group 25 cases.The control group were given the Seretide dry powder,1-2 times/day,and then hold breath after inhaling 10 to 15s,after that,rinsing with water to prevent oral fungal infection,and the treatment group were taken Guizhijiahoupoxingzi and Yupingfeng decoction（Guizhi 10g,Baishao 10g,Chuanpo 12g,Xingren 12g,Gancao 6g,Huangqi 20g,Fangfeng 12g,Baizhu 12g,Shengjiang 6g,Dazao 6g）,following volume ratio of 1： 5 water before soaking for 30 minutes,boiling 40min for 2 times,leaching it 2 times to about 200 ml,2 times a day will be taken every day.Both groups had 3 course of 10 day treatment（30 days）.Observe and record the clinical symptom and the poisonous side reaction.[Results]There are 7 cases cured,16 cases effective,2 cases invalid in the treatment group,and the total efficiency of 92.00%.In the control group,2 cases were cured,15 cases effective,8 cases invalid,the total efficiency of 68.00%.TCM syndrome score in both groups was significantly lower（P0.01）,and the treatment group was lower than the control group（P0.01）.[Conclusion]Guizhijiahoupoxingzi and Yupingfeng decoction can improve the clinical symptoms of COPD remission stage,and is effective and worthy of clinical application.
