针灸直刺联合宁神汤治疗肝火扰心型不寐随机平行对照研究

摘　　要：[目的]观察针灸直刺联合宁神汤治疗肝火扰心型不寐疗效。[方法]使用随机平行对照方法,将80例门诊患者按抛硬币方法随机分为两组,对照组40例泻肝安神胶囊,4粒/次,3次/d,治疗组40例①针灸直刺(采用35号2寸毫针针刺百会、四神聪、神庭、安眠双、风池双、太溪双、行间双),常规皮肤消毒后,施以直刺,手法用泻法,1次/d,留针20min/次。②宁神汤(黄芪40g,党参、当归、白术各20g,川芎15g,茯苓、白芍、生地、酸枣仁、柏子仁各20g,龙骨、牡蛎各25g,朱砂单包1g,柴胡20g,夏枯草15g,甘草10g),1剂/d,水煎取汁,早、午饭前20min和晚饭后8点,先将朱砂放入口中再用汤药送服,3次/d,连续治疗10d为1疗程。观测临床症状、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组痊愈20例,显效12例,有效7例,无效1例,总有效率97.50%。对照组痊愈11例,显效5例,有效3例,无效21例,总有效率47.50%。治疗组疗效优于对照组(P<0.01)。[结论]针灸直刺联合宁神汤治疗肝火扰心型不寐疗效满意。[Objective]Observe therapeutic effect of acupuncture piercing combining with ning-shen decoction in treating insomnia of liver-cardiac.[Method] Using randomized controlled methods,80 patients were random divided into two groups.40 cases of control group take Longdanxiegan capsules three times a day,four each while 40 cases of treatment group were accepted ①acupuncture piercing（adopt 35,No.2-inch needle acupuncture Baihui,Sishencong,Shenting,Anmian,Fengchi,Taixi,Xingjian）,after conventional skin disinfection,impose piercing with reducing method,once a day,needle 20 minute once;②ning-shen decoction（astragalus 40g,codonopsis 20g,angelica 20g,atractylodes 20g,chuanxiong 15g,poria 20g,white peony 20g,rehmanniae 20g,acid jujube nutlet 20g,Bozi 20g,keel 25g,oyster 25g,cinnabar 1g,bupleurum 20g,prunella 15g,licorice 10g）,one decoction a day,taking 20 minute before breakfastand lunch and eight P.M after supper,taking method is put cinnabar into the mouth first and then taking with decoction,three times a day,ten days is a course of continuous treatment.Observe clinical symptoms、adverse reaction.TO determine efficacy of three courses of continuous treatment.[Results]In the treatment group,20 cases were fully recovered,12 cases were significant valid,7 cases valid and 1 case noninvalid,total efficiency is 97.50% while 11 cases were fully recovered,5 were significant valid,3 valid,21 noninvalid in the control group,total efficiency is 47.50%.The efficacyis of treatment group is better than control group（P0.01）.[Conclusion]The efficacy of acupuncture piercing combining with ning-shen decoction in treating insomnia of liver-cardiac asthenia is satisfactory.
