温针灸循经筋辨证阿是穴治疗痉挛型脑瘫随机平行对照研究

摘　　要：[目的]观察温针灸治疗痉挛型脑瘫疗效。[方法]使用随机平行对照方法,将60例住院及门诊患者按就诊顺序编号法随机分成两组。均常规康复治疗。循经筋辨证阿是穴,运动区、平衡区穴、风池次、天髎次、臑会次、天宗次、阳池次、环跳次、阳陵次、足三里次、风市次、解溪次、华陀夹脊穴所分布竖脊肌分布区穴位。对照组30例传统针刺,治疗组30例温针灸(运动区、平衡区穴除外),留针30min,每次灸2段。1次/d,15次为1个疗程,疗程间休息2d,连续治疗3疗程,判定疗效。观测肌张力、屈肌伸肌肌力、腱反射、踝阵挛等情况、粗大运动功能测试量表(GMFM)总百分比、肌张力波动变化。[结果]治疗组显效18例,有效10例,无效2例,总有效率93.3%。对照组显效6例,有效17例,无效7例,总有效率76.7%。治疗组疗效优于对照组(P﹤0.05);GMFM总百分比提高,肌张力波动减轻(P<0.05),改善程度治疗组优于对照组(P<0.05)。[结论]温针灸在治疗痉挛型脑瘫方面效果显著,治疗组疗效优于对照组,值得在临床上广泛推广。[Objective]Observe the efficacy of the treatment of spastic cerebral palsy by Warm Acupuncture.[Method]60 patients were randomly divided into two groups.Contrast group of 30 patients in the conventional rehabilitation,on the basis of acupuncture,balance points area,the wind pool ci,Tianliao ci,Erhui ci,Tianzong ci,Yang Chi ci,Huantiao ci,Yang Ling ci,Tsusanli,Fengshi ci,Jiexi ci,and the Hua Tuo Jiaji distribution erector spine muscle distribution points.The treatment group of 30 patients on the basis of conventional rehabilitation warm acupuncture points（sports area,balance area points）.The needle for 30min,each moxibustion paragraph 2 times everyday,15 times as a course of treatment breaks up for 2 days,continuous treatment of 3 courses,in order to determine efficacy.Observations on muscle tone,flexor extensor muscle strength,tendon reflexes,ankle clonus,the total percentage of the Gross Motor Function Measure（GMFM）,and the fluctuation of muscle tension.[Results]The treatment group 18 remarkable valid,10 valid,2 invalid,the total efficiency of 93.3%.Contrasted group six cases markedly effect in 6,17 valid,and 7 invalid,the total efficiency of 76.7%.Treatment group is better than the contrasted group（P0.05）;totally increase in the percentage of children with GMFM,muscle tension fluctuation reduced（P0.05）,the degree of improvement treatment group is better than the contrasted group（P0.05）.[Conclusion]Warm acupuncture in the treatment of spastic cerebral palsy effect significant treatment group than the contrasted group,it is worth widely promoted in clinic.
