阴囊岛状皮瓣一期成形治疗2、3型先天性尿道下裂体会

摘　　要：目的总结阴囊岛状皮瓣一期重建尿道治疗先天性尿道下裂的手术经验,评估此术式在尿道下裂修复中的价值。方法回顾分析我院自2008年1月至2011年12月应用阴囊岛状皮瓣一期重建尿道治疗先天性尿道下裂46例患者的临床资料,其中阴囊阴茎型16例,阴茎型30例。结果阴囊阴茎型术后尿漏1例,无尿道狭窄患者,成功率93.7%;阴茎型术后尿漏3例,尿道狭窄1例,成功率86.7%。结论阴囊岛状皮瓣一期重建尿道治疗阴囊阴茎型先天性尿道下裂和阴茎型先天性尿道下裂成功率高,应是首选术式。保证阴囊中隔的系膜血管供养、选用合适的皮瓣宽度及分层次缝合是预防尿漏的关键。Objective To summarize the operation experience of scrotum island flap once reconstruction of congenital hypospadias,and evaluate the value of the operation in hypospadias repair.Methods Retrospective analysis 46 cases congenital hypospadias patients who accepted scrotum island flap once reconstruction operation of our hospital from January 2008 to December 2011.There were 16 cases of Scrotal penis type,and 30 cases of penis type.Results 1 case urinary leakage in scrotal penis,and none urethral stricture,the success rate of 93.7%.3 cases urinary leakage in Penis type,and 1 case happened urethral stricture,the success rate of 86.7%.Conclusions Scrotum island flap once reconstruction of Scrotal penis type and penis type congenital hypospadias,was high success rate,should be preferred.To certify essential of scrotal septal vascular support,choose suitable flap width and layer suture is the key to the prevention of urinary leakage.
