综合护理干预对尿道下裂术后并发症的影响

摘　　要：目的探讨综合护理干预对预防尿道下裂术后尿瘘、尿道狭窄及感染发生的效果。方法将160例尿道下裂术后患儿随机分为观察组和对照组两组,对照组(80例)接受术后常规护理。观察组(80例)在常规护理基础上,采用术前3天用日舒安洗液1∶20浸泡会阴部,手术中采用优拓包扎阴茎创口,术后积极予镇痛抗勃起通便措施,并用生理盐水5 ml加庆大霉素8万U从尿道支架管滴入,生理盐水冲洗尿管保持尿管通畅,用0.5%碘伏加生理盐水湿润阴茎敷料拆除加压包扎阴茎敷料,然后予红外线照射阴茎切口。结果观察组与对照组比较尿道下裂术后患儿尿瘘、尿道狭窄的发生,差异有统计学意义(P<0.05)。结论通过综合护理干预可显著减少尿道下裂术后尿瘘、尿道狭窄及感染的发生。Objective To investigate the effect of comprehensive nursing intervention on the prevention of postoperative urinary fistula,urethral stricture and infection.Methods 160 cases of hypospadias surgery were randomly divided into two groups,observation group and control group,the control group（80 cases） received routine nursing after operation.The observation group（80 cases） on the basis of conventional nursing,the 3 day before operation with risuan lotion 1： 10 immersion of perineum,used Urgotul dressing penile wound,postoperative analgesia actively to anti erectile defecation measures,and 5 ml plus gentamicin 80000 U from urethral stent instilled with saline,saline irrigation catheter to maintain catheter patency,with 0.5% iodophor plus saline moist dressing removal of pressure bandage dressing penis,and then to the infrared radiation penis incision.Results Observation group compared with control group,the incidence of Children with urinary fistula,urethral stricture after operation,the difference was statistically significant（P 0.05）.Conclusions The comprehensive nursing intervention can significantly reduce the incidence of Urinary fistula,urethral stricture and infection after operation.
