早期连续性血液净化治疗中毒性横纹肌溶解症效果观察

摘　　要：目的探讨早期持续性血液净化治疗横纹肌溶解症(rhabdomyolysis,RM)的临床效果。方法选择我院2006年3月～2010年12月抢救的中毒性横纹肌溶解患者31例,进行回顾性分析,分析患者病情变化规律,探讨连续性肾脏替代治疗CRRT的时机及对预后影响。观察肌红蛋白,肌酸磷酸激酶,乳酸脱氢酶,肌酐,尿素氮等的变化。结果 31例患者中,6例因发生多器官衰竭死亡,其余痊愈出院。其中,12例患者在发病初期即进行CRRT,减少了并发症发生,改善预后,提高生存率。结论早期连续性血液净化治疗横纹肌溶解疗效显著,可以有效清除肌红蛋白,不仅降低住院时间和死亡率,也能有效减少肾功能不全的发生。Objective Study the clinical effects of early hemofiltration treatment of rhabdomyolysis.Methods Select toxic rhabdomyolysis 31 patients in March 2006 to December 2010 in our hospital to analysis retrospectively.Analyze the patient’s condition variation,discuss the timing of treatment and impact on prognosis of CRRTand observe the changes in myoglobin,creatine kinase,lactate dehydrogenase,creatinine,blood urea nitrogen,etc.Result In 31 patients,6 deaths due to multiple organ failure,and the rest were discharged.12 patients perform CRRT in the early stages of disease so as to reduce the incidence of complications,improve outcomes,improve survival.Discusiion Early continuous blood purification have significant effect on the treatment of rhabdomyolysis.And CRRTCan effectively remove myoglobin,not only to reduce the length of hospital stay and mortality,can be effective in reducing the incidence of renal insufficiency.
