摘  要:目的探讨膜性肾病患者蛋白尿对肾小管上皮细胞转分化的影响。方法提取膜性肾病患者尿蛋白进行蛋白成分分析、细菌内毒素试验、蛋白溶液的肌酐水平等质量鉴定后,尿蛋白组分别按1.0、2.0、4.0、8.0 mg/mL蛋白质量浓度刺激人肾小管上皮细胞(hRTEC)48 h;空白对照组无尿蛋白,采用蛋白质印迹法(Western blot)检测hRTEC胞浆内平滑肌肌动蛋白-α(α-SMA)的表达,观察hRTEC转分化情况。结果膜性肾病患者尿蛋白组分构成:清蛋白为56.4%、转铁蛋白为34.6%、免疫球蛋白G(IgG)为6.9%。膜性肾病患者尿蛋白诱导hRTEC胞浆中α-SMA水平随蛋白质量浓度的增加而增加(P<0.05)。结论膜性肾病患者的蛋白尿是导致肾间质纤维化的原因之一,并具有质量浓度依赖关系。Objective To explore the effects of proteinuria on renal interstitial cell fibrosis in the patients with membranous nephropathy(MN).Methods Urinary protein was extracted from the patients with MN for conducting the protein composition analysis,bacterial endotoxin test and quality identification of creatinine level in the protein solution.Then human renal tubular epithelial cells(hRTEC) were respectively stimulated by urinary protein with the concentration(the experiment group) of 1.0,2.0,4.0,8.0 mg/mL for 48 h;the blank control group had no urinary protein,the Western blot was adopted to test α-smooth muscle actin(α-SMA) expression in hRTEC intracytoplasm and the transdifferentiation of hRTEC was observed.Results The component composition of urinary protein in the patients with MN:albumin 56.4%,transferrin 34.6% and IgG 6.9%.The intracytoplasm α-SMA level of patients with MN in hRTECs induced by urinary protein was increased with the increase of the protein mass concentration(P0.05).Conclusion Proteinuria in the patients with MN is one of the causes inducing renal interstitial fibrosis,which shows the mass concentration dependence relation.
