摘 要:目的探讨BACTEC FX全自动血液培养系统对血液标本培养中细菌感染种类及构成情况的评估分析,以评价该系统的临床应用价值。方法采用BACTEC FX全自动血液培养系统对4 215份血液标本分别进行需氧、厌氧菌培养,对细菌检出阳性时间、阳性率、假阳性、假阴性以及种类进行评估分析。结果血液标本中最快阳性检出时间为2.5 h,12、24、48、72 h阳性检出率分别为24.6%(126/513)、64.1%(329/513)、79.1%(406/513)、94.7%(486/513);分离出16属、40种、513株病原微生物,阳性率为12.2%(513/4 215),其中革兰阳性球菌287株(55.9%),革兰阴性杆菌203株(39.6%),二者占总检出率的95.5%(490/513);假阳性率为1.6%(8/513),假阴性率为0.3%(11/3 702)。结论 BACTEC FX全自动血液培养系统临床应用能提高病原菌检出阳性率,缩短阳性检出时间,检出细菌种类多,能快速、准确地为临床提供败血症及严重感染诊断和治疗的可靠依据。Objective To explore the influence of the BACTEC FX fully automatic blood culture system on the types and constituent status of bacterial infection in blood culture specimens for evaluating its clinical application value.Methods The BACTEC FX fully automatic blood culture system was adopted to conduct aerobic cultivation and anaerobic cultivation on 4 215 blood specimens respectively.The bacterial positive detection time,positive rate,false positive,false negative and types were evaluated and analyzed.Results The fastest positive detection time in blood specimens was 2.5 h.The positive detection rate at 12,24,48,72 h was 24.6%(126/513),64.1%(329/513),79.1%(406/513) and 94.7%(486/513) respectively.16 genera,40 species,513 strains of pathogenic microorganisms were isolated,with the positive rate of 12.2%,including 287 strains of Gram-positive cocci(55.9%),203 strains of Gram negative bacteria(39.6%),both accounting for 95.5%(490/513) of the total detection rate.The false positive rate was 1.6%(8/513) and the false negative rate was 0.3%(11/3 702).Conclusion The clinical application of the BACTEC FX fully automatic blood culture system can improve the positive rate of pathogenic bacterial detection,shorten the time of positive detection,detect more bacterial species,and quickly and accurately provide the reliable basis for the diagnosis and treatment of septicemia and serious infections.
