肿瘤科感染患者血培养病原菌分布特点及药敏分析

摘  要:目的了解肿瘤科感染患者血培养病原菌分布及对抗生素敏感和耐药情况,为临床合理用药提供依据。方法对我院肿瘤科891例感染患者发热期或者有感染症状的血培养标本进行病原菌培养和对抗生素敏感情况进行回顾性分析。结果普通血培养891例,细菌检出129例(14.48%),其中革兰氏阳性球菌占40.3%,以金黄色葡萄球菌、凝固酶阴性葡萄球菌为主;革兰氏阴性杆菌占45%,以大肠埃希菌、肺炎克雷伯菌为主;真菌占14.7%,以白色念珠菌为主。革兰氏阴性杆菌对美罗培南和亚胺培南敏感率为100%、对TMP-SMZ耐药率为98.3%;革兰氏阳性球菌对亚胺培南、万古霉素、替考拉宁敏感率为100%、对TMP-SMZ、青霉素耐药率为100%;真菌对伏立康唑敏感率为100%。结论临床应当重视肿瘤科感染患者血培养,应根据病原菌的敏感性试验合理使用抗生素。Objective To study the blood culture pathogens of infected patients from oncology department, and discuss the susceptible and resistant to antibiotics of the pathogens,to provide the basis for clinical rational use of drugs.Methods A retrospective analysis of blood culture pathogens and their susceptible and resistant to antibiotics was done in 891 infected patients with fever or have symptoms of infection from oncology department. Results Among 891 blood cultures,bacteria were detected in 129 cases(14.48%).Gram-positive cocci accounted for 40.3%,mainly Staphylococcus aureus and coagulase-negative staphylococcus.Gram-negative bacilli accounted for 45%,mainly Escherichia coli,Klebsierlla pneumoniae.Fungi accounted for 14.7%, mainly Candida albicans.Among Gram-negative bacilli,the susceptible rates of Meropenem and imipenem were 100%,the resistance rate of TMP-SMZ was 98.3%.Among Gram-positive cocci,the susceptible rates of imipenem, vancomycin and teicoplanin were 100%,the resistance rates of penicillin and TMP-SMZ were 100% .Among fungi,the susceptible rate of voriconazole was 100%.Conclusion The blood culture of infected patient should be emphasized in clinical work,the rational use of antibiotics should be based on the susceptibility test of pathogens.
