急性高容量血液稀释联合硝酸甘油控制性降压用于子宫内膜癌根治手术临床疗效研究

摘　　要：目的观察急性血液稀释联合硝酸甘油控制性降压用于子宫内膜癌根治手术的效果。方法选择A、B、C三组病人手术开始前A.B组输注乳酸林格氏液及6%的羟乙基淀粉(贺斯),C组输入乳酸林格氏液10 ml/kg及羟乙基淀粉10 ml/kg,术中连续临测MAP,HR,ECG,CVP,PETC02及血氧饱和度(SPO_2),并记录术前T1,术后1 hT_2及术后24 h T_3的Hb,Hct,血小板(Plt)和PH值。结果 C组病人CVP与术前比较有升高但无显著性差异,A B两组病人CVP无明显变化。结论急性高容量血液稀释复合控制性降压用于子宫内膜癌根治手术病人可明显减少术中出血量,是一种安全有效的临床血液保护方法,值得推广。Objective Observes the acute hypervolaemic haemodilution union Nitroglycerin controllability to fall the pressure to use in the endometrial carcinoma radical operation effect.Methods A,B,C three groups of patients surgery before the start of the the group AB infusion of Ringer＇s lactate and 6%hydroxy ethyl starch（HES）,Group C input lactated Ringer＇s solution of 10 ml/kg and the hydroxyethyl starch 10 ml/kg,intraoperative continuous clinical testing MAP,HR,ECG,CVP,PETCO_2 and blood oxygen saturation（SPO2）, And record preoperative T1,1 hour after T2 and 24 hours after the T3 of Hb,Hct,platelet（Plt） and PH value. Results Group C patients CVP preoperative elevated but no significant difference was no significant change in AB patients CVP.Conclusion Acute hypervolemic hemodilution compound induced hypotension for endometrial cancer patients with radical surgery can significantly reduce blood loss,is a safe and effective the clinical blood conservation method,worthy of promotion.
