摘 要:目的探讨超声导航在颅内病变微创手术治疗中的作用。方法 40例经影像学诊断为颅内病变患者于开颅后应用超声探测颅内病变部位、范围、内部结构及毗邻关系,确定颅内病变微创手术入路,指导术中安全微创切除病变。结果术中40例位于颅内的病灶均在超声图像显示,准确定位;术后组织病理证实脑胶质瘤25例,表现为不均匀的强回声,多数边界不清晰;7例海绵状血管瘤表现为边界清晰的强回声,中心多呈蜂窝样改变;转移性肿瘤8例,表现为高回声,形态规则,境界清晰,回声均匀。结论术中超声作为一种实时、无创的导航工具,可准确定位颅内病变,明确其血液供应及周围结构,有助于提高手术治疗效果。Objective To investigate the application of ultrasound navigation in minimally invasive surgical treatment of intracranial lesions. Methods Ultrasound was used to with neighbors of intracranial lesions after craniotomy measure the location, scope, internal structure and the relationship in 40 cases of imaging diagnosed intracranial lesions, to determine minimally invasive surgical approach for intracranial lesions and guide the safe removal of lesion. Results All these 40 cases of intracranial lesions were displayed dearly on ultrasound and localized accurately. Postoperative pathology confirmed glioma in 25 cases demonstrating strong uneven echo and unclear boundary, cavernous hemangioma in 7 cases demonstrating strong echo with clear boundary and honeycomb-like center, and metastatic tumor in 8 cases demonstrating high echo, regular configuration, clear boundary and even echo. Conclusion The intraoperative ultrasound, a real-time, noninvasive navigation tool, can accurately localize intracranial lesion and discover the blood supply and neighbor configuration so as to improve the therapeutic effect.
