摘  要:目的确定符合天津市Graves病患者促甲状腺激素受体抗体(TRAb)的诊断灰区。方法通过电化学发光法检测Graves病组(n=157)、甲状腺炎组(n=250)以及正常对照组(n=235)的TRAb血清水平,绘制受试者工作特征(ROC)曲线,计算Graves病最佳诊断切值并确定诊断灰区。结果区分Graves病患者和健康人的TRAb最佳诊断切值为1.725IU/L,此时ROC曲线下面积为0.996,敏感度为97.5%,特异度为99.6%。区分Graves病患者和甲状腺炎患者的诊断灰区为1.670—2.535IU/L。结论确定Graves病患者TRAb诊断灰区有卧干杯高诊断特异一件和钎戚件。Objective To evaluate the diagnostic gray zone of thyrotropin receptor antibody (TRAb) in patients with Graves' disease in Tianjin. Methods Chemiluminescence method was applied to detect the serum levels of TRAb in Graves' disease group ( n = 157 ), thyroiditis group ( n = 250 ) , and normal control group ( n = 235 ). The optimal diagnostic cutoff value was calculated and diagnostic gray zone was determined by receiver operating characteristic (ROC) curve. Results ROC analysis resulted in a specificity of 99.6% with a sensitivity of 97.5% at cutoff value of 1. 725 IU/L with area under the ROC curve is 0.996 from com-parison of Graves' disease and healthy volunteers. The diagnostic gray zone of distinction between Graves' disease and thyroiditis was 1. 670-2.535 IU/L. Conclusion To evaluate the diagnostic gray zone of TRAb in oatients with Graves' disease is very helpful for improving the specificity and the sensitivity of the diagnosis.
