双ENDOBUTTON CL钛板与锁骨钩钛板治疗Tossy Ⅲ型肩锁关节脱位的临床疗效比较

摘　　要：目的比较双ENDOBUTTON CL钛板与锁骨钩钛板治疗TossyⅢ型肩锁关节脱位的临床疗效。方法61例TossyⅢ型肩锁关节脱位患者,采用切开复位锁骨钩钛板内固定术治疗31例,采用切开复位ENDOBUTTON CL系统内固定术治疗30例。按Karlsson疗效标准对两组进行肩关节功能评定。结果61例患者均得到随访,时间8～28月,平均(12.4±2.6)月,脱位全部复位,无再脱位。均恢复正常生活和工作。按照Karlsson标准评定患者术后的肩关节功能恢复情况:锁骨钩钛板固定组:优11例,良9例,可7例和差4例,优良率64.52%;双ENDOBUTTON CL钛板固定组:优15例,良11例,可3例和差1例,优良率86.67%,两组疗效的优良率比较差异有统计学意义(x^2=4.0340,P<0.05)。结论双ENDOBUTTON CL钛板治疗TossyⅢ型肩锁关节脱位的效果好于锁骨钩钛板,具有对肩锁关节功能干扰较小,术后肩痛发生率低、内固定不必取出等优点。Objective To compare the resuh of treatment on Tossy Ⅲ acmmioclavicular dislocation betweenwith double ENDOBUTI＇ON CL plate and clavicular hook plate. Methods 61 patients with Tossy m type acromio- clavieular dislocation were treated with open reduction and internal fixation. Out of the 61 cases, 31 cases were treated with clavicular hook, 30 cases with ENDOBUTI＇ON CL system. Results These 61 patients were followed up from 8 to 28 months （averaged 12.4 ± 2.6 months）. All acmmioclavicular joint dislocation was reduced. Ac- cording to the scales of Karlsson, the post- operation shoulder- joint function were excellent in 11 patients, good in 9 cases, satisfactory in 7 cases and poor in 4 cases in the clavicular hook group. The rate of both excellent and good were 64.52%; the post- operation shoulder-joint function were excellent in 15 cases, good in 11 cases, average in 3 cases and poor in 1 cases in the ENDOBUTIDN CL System group. The rate of both excellent and good were 86.67 %. The difference of the rate of both excellent and good had statistical significance between the two groups（ x2 = 4.0340, P 〈 0.05）. Conclusion In the treatment of Tossy Ⅲ acromioclavicular dislocation, fixation of double ENDOBUTFON CL plate is better than hook plate fixation.
