快速康复外科模式在胆囊切除术病人中的应用

摘　　要：[目的]探讨在胆囊切除术病人中应用快速康复外科模式,不置胃管的安全性及可行性。[方法]选择2010年12月—2012年10月在我院行胆囊手术的病人81例,随机分为快速康复外科组40例和对照组41例,快速康复外科组应用快速康复外科模式,术前不置胃管,术后早期恢复经口进食,术前对病人进行健康教育;对照组术中常规安置胃管,直至术后恢复肛门排气后拔除。比较两组术后恶心、呕吐、肺部感染、咽喉疼痛的发生率及肛门排气时间的差异。[结果]两组病人术后恶心、呕吐、肺部感染、咽喉疼痛的发生率比较差异有统计学意义(P<0.05);两组肛门排气时间比较差异无统计学意义(P>0.05)。[结论]应用快速康复外科模式在胆囊手术病人不置胃管并早期恢复经口进食是安全可行的,可以减少病人不适感并有利于病人快速康复。Objective：To probe into the safety and feasibility of non-setting gastric tube for fast track surgery mode applied for cholecystectomy patients.Methods：A total of 81 cases of patients undergoing gallbladder surgery in our hospital from December in 2010 to October in 2012 were selected and randomly divided into fast track surgery group with 40 patients and the control group with 41 patients,the patients in fast track surgery group used fast track surgery mode,including non-setting gastric tube before operation,early recovery of postoperatively eating by mouth for patients,preoperative health education for patients;intraoperative placement of gastric tubes for patients in control group,and the gastric tubes were removed until postoperative recovery of flatus,then to compare the differences in postoperative nausea,vomiting,pulmonary infection,the incidence of sore throat and anal exhaust time between two groups.Results：There was statistically significant difference in incidences of postoperative nausea,vomiting,pulmonary infection and sore throat of patients between both groups（P0.05）;there was no statistically significant difference in anal exhaust time between both groups（P0.05）.Conclusion：It is safe and feasible to apply the fast track surgery mode for early recovery of oral feeding of patients undergoing gallbladder surgery who did not set the gastric tube,and it can reduce patients＇discomfort and facilitate rapid rehabilitation of patients.
