大剂量沐舒坦联合补肾益肺化瘀汤在老年慢性阻塞性肺病肺部感染气管插管患者中临床应用

摘　　要：目的:研究大剂量沐舒坦联合补肾益肺化瘀汤在老年慢性阻塞性肺病肺部感染气管插管患者中的临床应用价值。方法:选择老年慢性阻塞性肺病并发肺部感染患者作为研究对象,随机分为在气管插管后给予沐舒坦联合补肾益肺化瘀汤治疗的观察组和给予沐舒坦雾化吸入的对照组,观察肺部感染恢复情况、咳嗽反应情况以及生活质量情况。结果:观察组患者插管后3 d起CPIS评分明显低于治疗前,恢复快于对照组;拔管后的平均咳嗽次数明显少于对照组,KPS评分、躯体功能、心理功能、社会功能、认知功能和总体生活质量评分均明显高于对照组。结论:沐舒坦雾化吸入联合补肾益肺化瘀汤能够有效的改善临床相关指标,对于慢性阻塞性肺病肺部感染气管插管患者具有积极的临床价值。Objective ： To study the effects of large dose of ambroxol and Bushen Yifei Huayu Decoction in elderly patients with COPD pulmonary infection treated with tracheal intubation. Methods ： Elderly patients with COPD pulmonary infection treated with tracheal intubation were collected and randomly divided into observation group given Mucosolvan aerosol inhalation and Bushen Yifei Huayu Decoction, while lung control group given Mueosolvan aerosol inhalation. Then recovery of pulmonary infection, cough response and quality of life were observed. Results ： CPIS score of observation group was significantly lower from 3 d after intubation, which was faster than that of control group ; average cough frequency was significantly less than that of control group; KPS score, body function, mental function, social function, cognitive function and the general quality of life score were significantly higher than those of control group. Conclusion ： Mucosolvan aerosol inhalation and Bushen Yi^ei Huayu Decoction has positive clinical value for it earl effectively improve the clinical correlation index.
