血清心脏型脂肪酸结合蛋白对2型糖尿病患者早期心肌损害的监测价值

摘　　要：目的探讨血清心脏型脂肪酸结合蛋白(h-FABP)水平对2型糖尿病(T2DM)患者早期心肌损害的监测价值。方法选择我院住院和门诊临床确诊的T2DM患者260例,将其按照病程分为<10年组(185例)和≥10年组(75例),并选择同期体检健康者50例作为对照组。检测各组血清h-FABP水平,并进行对比分析。结果T2DM患者血清h-FABP水平高于对照组,差异有统计学意义(P<0.01);≥10年组患者血清h-FABP水平高于<10年组,差异有统计学意义(P<0.01);≥10年组患者血清h-FABP阳性率高于<10年组,差异有统计学意义(P<0.01)。104例血清h-FABP阳性T2DM患者中,冠状动脉造影心脏病变发生率(35.8%)与血清h-FABP阳性率(40.0%)比较,差异无统计学意义(P>0.05)。结论 T2DM患者体内存在血清h-FABP水平的异常表达,与患者病程相关,检测血清h-FABP水平,可作为早期心肌损害的一项监测指标。Objective To study the monitoring value of serum heart-type fatty acid binding protein（h-FABP） levels on early myocardial damage in patients with type 2 diabetes mellitus（T2DM）.Methods In 260 T2DM patients in our hospital inpatient and outpatient clinical diagnosed cases,according to the course is divided into ＆lt; 10 years group（185 cases） and ≥ 10 years group（75 cases）,and 50 healthy persons as control group.The levels of serum h-FABP were detected,and carries on the contrast analysis.Results Serum h-FABP level in patients with T2DM than in the control group,the difference was statistically significant（P ＆lt; 0.01）;≥10 years group patients with higher serum h-FABP level than ＆lt; 10 years group,the difference was statistically significant（P ＆lt; 0.01）;≥10 years group the serum h-FABP positive rate was higher than that of ＆lt; 10 years group,the difference has statistical significance（P ＆lt; 0.01）.In 104 patients with h-FABP positive T2DM patients,cardiac angiographic coronary artery disease incidence rate（35.8%） and serum h-FABP positive rate（40.0%） compared,there was no significant difference（P ＆gt; 0.05）.Conclusion T2DM patients exist in serum h-FABP level of abnormal expression,and patients’ course related to the level of serum h-FABP,which can be used as early myocardial injury,a monitoring indicators.
