硒对甲状腺功能正常的桥本甲状腺炎的影响研究

摘　　要：目的观察补硒治疗对甲状腺功能正常的桥本甲状腺炎的干预效果。方法将伴有甲状腺肿大的甲状腺功能正常的94例桥本甲状腺炎患者按随机数字表法分为硒治疗组(48例)和对照组(46例),观察两组治疗前和治疗6个月后的甲状腺各径线及结节大小、甲状腺功能及甲状腺过氧化物酶抗体(TPO-Ab)、甲状腺球蛋白抗体(TGAb)、糖化血红蛋白(HbA1c)水平的变化。结果治疗前两组各观察指标比较,差异均无统计学意义(P>0.05)。治疗后硒治疗组TGAb、TPO-Ab水平均较对照组降低,差异有统计学意义(均P<0.05);硒治疗组的甲状腺各径线及结节大小均较对照组缩小,差异有统计学意义(P<0.05)。结论补硒治疗对甲状腺功能正常的桥本甲状腺炎患者的甲状腺缩小、结节缩小及自身抗体水平的降低是有益的,而对患者的甲状腺功能和HbA1c水平无影响。Objective To observe the influence of selenium therapy on Hashimoto Thyroiditis（HT） with normal thyroid function.Methods 94 HT patients with thyroid enlargement and normal function were randomly divided into selenium therapy group（48 cases） and control group（46 cases）.The size of thyroid and nodules,thyroid function,thyroid peroxidase antibody（TPO-Ab）,thyroglobulin antibody（TGAb） and glycated hemoglobin（HbA1c） of the two groups were compared before and after six months treatment.Results The above mentioned indexes before treatment showed no statistically significant difference between the two groups（P《0.05）.After treatment,the TGAb and TPO-Ab levels in the selenium therapy group were significantly lower than the control group（P《0.05）.The size of thyroid and nodules before treatment showed no statistically significant difference between the two groups（P《0.05）,but were significantly smaller in the selenium group after treatment compared with control group（P《0.05）.Conclusion The supplementation therapy of selenium on HT patients with normal thyroid function may be beneficial to reduce the size of thyroid and nodule,lower the level of autoantibodies,but it shows no obvious influence on thyroid function and HbA1c.
