D-dimer、P-selectin、ACA与下肢深静脉血栓形成和复发的研究

摘　　要：目的探讨血D-二聚体(D-dimer)、P-选择素(P-selectin)及抗心磷脂抗体(ACA)与下肢深静脉血栓形成和复发的关系。方法选择100例下肢深静脉血栓患者作为观察组,其中初发患者71例,复发患者29例,另外选择100例健康人作为对照组,测定所有人血D-二聚体、P-选择素及抗心磷脂抗体,均采用酶联免疫吸附双抗体夹心法(ELISA)测定。结果对照组的血D-二聚体、P-选择素和抗心磷脂抗体阳性分别为(171.2±37.3)ng/ml,(15.3±8.6)μg/L和15例;观察组血D-二聚体、P-选择素和抗心磷脂抗体阳性分别为(397.5±49.1)ng/ml,(31.5±10.9)μg/L和56例;观察组血D-二聚体、P-选择素和抗心磷脂抗体阳性均明显高于对照组(P<0.01);再发组的血D-二聚体、P-选择素和抗心磷脂抗体阳性均明显高于初发组(P<0.05)。结论血D-二聚体、P-选择素及抗心磷脂抗体与下肢深静脉血栓形成和复发有密切关系,可以作为其监测指标。Objective To study the correlation of serum D-dimer, P-selectin, anticardiolipin antibodies （ACA） with deep vein thrombosis （DVT） and relapse. Methods One hundred patients with DVT（ first diagnosed cases 71 and relapse cases 29） were enrolled into the observation group and one hundred healthy people into the control group. The levels of serum D-dimer, P-selectin and ACA were detected by ELISA. Results The levels of Serum D-dimer and P-seleetin were （ 171.2 ~ 37.3 ） ng/ml and （ 15.3 ＋ 8.6） txg/L in the control group, respectively ; and （ 397.5 ＋ 49.1 ） ng/ml and （ 31.5 -＋ 10.9 ） p^g/L in the observation group, respectively. The positive cases for ACA were 15 in the control group and 56 in the observation group. Serum D-dimer, P- selectin and ACA in the observation group were higher than these in the control group （P 〈 0.01 ）. Serum D-dimer, P-selectin and ACA in relapse cases were higher these in first diagnosed cases （P 〈 0.05 ）. Conclusion There was a positive correlation be- tween the levels of serum D-dimer,P-selectin and ACA and deep vein thrombosis. They can be used as monitoring index of deep vein thrombosis.
