S100B蛋白浓度的测定对颅脑损伤分型与判断预后的意义

摘　　要：目的探讨TBI患者血清S100B蛋白水平的变化,并分析其与损伤严重程度及预后的相关性。方法入选200例TBI患者,根据入院时GCS评分进行分组、并选择30例健康体检者作为正常对照组,测定伤后24 h、48 h、72 h、120 h血中S100B蛋白浓度。分析血清S100B蛋白浓度水平,与TBI入院时GCS评分,及患者伤后1个月GOS评分的相关性。结果 TBI组血清中S100B蛋白浓度较对照组明显升高(P<0.05),不同程度TBI组在同一个时间点之间比较,S100B蛋白浓度差异有统计学意义(P<0.05)。血清S100B蛋白含量与TBI严重程度成正相关,与GCS和GOS评分成负相关(r=0.784,P<0.05;r=-0.835,P<0.05;r=-0.793,P<0.05);TBI患者恢复不良组58例,恢复良好组142例,恢复不良组与良好组之间血清S100B蛋白浓度差异具有统计学意义(P<0.05)。血清S100B蛋白含量越高,TBI越严重,预后越差。结论血清S100B蛋白测定可作为判断TBI严重程度及预后的重要指标,具有重要的临床应用价值。Objective To study traumatic brain injury patients blood S100B protein level of change, and analyze the relevance of which with injury severity and the prognosis. Methods Two hundred TBI patients were admitted, which were divided in- to four groups according to the GCS of admission, and select 30 cases of healthy physical examination as normal control, the S100B protein concentration was detected after injuring 24 h,48 h,72 h, 120 h, the pearson correlation was studied for the S100B protein concentration with the admission GCS score and the GOS score after injuring for one month. Results The serum S100B of TBI group was sharply riser than the control group, and had the statistics difference （ P 〈 0.05 ）, The different degree of TBI group in the same time point was compared, the difference of S100B protein concentration had statistical significance（ P 〈 0.05 ）. Serum S100B protein content was positively correlated with the severity of TBI,and negatively correlated with GCS and GOS score（ r = - 0.784, P 〈 0.05 ; r = - 0. 835, P 〈 0.05 ; r = - 0. 793, P 〈 0.05 ） ; TBI patients with poor recovery group were fifty eight cases, good recovery group were one hundred forty two cases,poor recovery group and good recovery group between serum S-100B pro- tein concentration has significant differences. Namely serum S100B protein concentration was the higher, the more serious for TBI injury,the worse for the prognosis. Conclusion The determination of serum S100B protein concentration can be used as the important index for judgment patients with TBI for the severity and the prognosis, which had the important clinical value.
