摘  要:目的通过对比研究基于FSE的3D Spiral采集动脉自旋标记(3D-ASL)法与动态磁敏感对比增强(DSC)法,探讨3D-ASL在颅脑肿瘤中的应用价值。方法选择39例经病理或临床及影像学证实的颅脑肿瘤患者,脑内肿瘤24例,脑外肿瘤15例。行常规MRI及3D-ASL、DSC检查。将轴位增强图像与两种技术所得功能图融合,测量肿瘤灌注最明显区、水肿区及健侧CBF值,计算TBFmax/CBF健侧比值,将结果做配对t检验。结果 36例两种技术肿瘤区灌注表现一致,3例3D-ASL广泛显著高灌注而DSC法极少高灌注。3D-ASL法TBFmax/CBF健侧为2.65±2.63,DSC法为2.66±2.13,t=-0.029,P=0.977,差异无统计学意义。结论 3D-ASL与DSC法都可量化地反映颅脑肿瘤内部微血管密度和血管分布情况,其结果高度符合,而3D-ASL安全无创,不用对比剂,在颅脑肿瘤中有着广泛的应用前景。Objective To compare the effect of three-dimensional arterial spin labeling(3D ASL) and dynamic-susceptibility contrast-enhanced (DSC) MRI on diagnosis of brain tumor. Methods Total 39 cases of intracranial tumors(24 brain tumors and 15 tumors outside the brain) confirmed by pathology or clinical and radiographic evidence underwent conventional,3D-ASL and DSC MRI. The function graphs obtained by 3D-ASL and DSC were merged with the axial enhanced images to measure the area with high perfusion and edema in tumor, and the contralateral CBF. TBFmax/CBF contralateral ratio was calculated and tested with paired t test. Results The tumor perfusion by 3D-ASL and DSC were consistent in 36 cases. The high perfusion by 3D-ASL but not by DSC was found in 3 cases. 3D-ASL showed that TBFmax/CBF were 2.65 ~2.63 in healthy side and DSC showed that it was 2.66 ~ 2.13 in. There was no significant difference between the two methods ( t = -0.029, P = 0. 977 ). Conclusion Both 3D-ASL and DSC MRI, with a highly consistent, can quantitatively reveal the microvessel density and vascular distribution in brain tumors. Because no contrast agent is employed,3D-ASL method is safer and noninvasive,and has a broad application pros- pect in brain tumors.
