SLIPA喉罩与ProSeal喉罩用于腹腔镜手术中安全性的Meta分析

摘　　要：目的采用Meta分析评价SLIPA喉罩和ProSeal喉罩(PLMA)应用于腹腔镜手术的安全性。方法计算机检索PubMed、EMbase、Ovid Medline、SCI数据库、CNKI数据库、中国生物医学文献数据库、万方数据库,收集1998～2012年SLIPA喉罩和PLMA的随机对照试验(RCT),按Cochrane系统评价的方法对纳入研究进行资料提取及质量评估后,采用RevMan 5.0软件进行分析。结果共纳入7个RCT,577例患者。Meta分析结果显示:(1)SLIPA喉罩的置管时间长于PL-MA[WMD=1.8s,95%可信区间(CI)0.80～2.83];(2)术后喉罩沾血率:SLIPA喉罩明显高于PL-MA(RR=1.79,95%CI1.13～2.82);(3)SLIPA喉罩与PLMA的首次置管成功率、密封压、术后咽痛率和反流率差异无统计学意义。结论 SLIPA喉罩与PLMA比较,在安全性方面无明显差别;放置SLIPA喉罩首次成功率未提高,但是花费的时间比PLMA长。Objective To evaluate the effectiveness and safety of streamlined liner of the pharynx airway（SLIPA） compared to ProSeal laryngeal mask airway （PLMA）for airway management in the patients undergoing laparoseopic surgery. Methods The databases were Pubmed, EMbase, Ovid Medline, Science Citation index, China Academic Journal Network Publishing, CBM, Wanfang from the year of 1998 to 2012 were searched to indentify randomized controlled trials（RCTs）about the SLIPA and PLMA, and the relevant references of the included papers were also manually searched. Two reviewers independently screened the trials according to inclusion and exclusion criteria, extracted the data, and assessed the methodology quality. Meta-analysis were performed using the Cochrane Collaboration＇s RevMan 5.0 software. Results Seven RCTs involved 577 patients were included. Meta-analysis results showed that： （1）The insertion time of SLIPA was longer than PLMA（WMD= 1.8 s, 95% CI 0. 80-2. 83 ） （2）The incidence of laryngeal mask airway with blood stain： was signifacantly higher in SLIPA than PLMA（RR= 1.79,95%CI 1.13-2.82）（3） There was no statistic difference of first insertion success rate, airway sealing pressure, incidence of postoperative sore throat between SLIPA and PLMA. Conclusion SLIPA revealed no superiority of airway management than conventional PLMA.
