摘  要:目的观察新生大鼠不同剂量丙泊酚麻醉对成年后认知功能及海马特异性核蛋白(NeuN)表达的影响。方法雄性SD大鼠100只,日龄7d,体重9~18g,随机分为对照组(C组)、丙泊酚25mg/kg组(P1组)、丙泊酚50mg/kg组(P2组)、丙泊酚100mg/kg组(P3组)和丙泊酚200mg/kg组(P4组),每组20只。P1组和P2组单次腹腔注射丙泊酚25或50mg/kg;P3组和P4组首次注射丙泊酚50mg/kg,待大鼠有体动反应(40~60min)后,每次再追加丙泊酚50mg/kg,直至给完总量。每组取5只大鼠,苏醒后即刻抽取动脉血样进行血气分析。其余大鼠苏醒后放回笼内继续饲养直至9周龄,Morris水迷宫测试大鼠空间学习记忆能力,定位航行实验测试第1天~第6天大鼠登上平台的时间(逃避潜伏期)。免疫组化法和Westernblot法检测大鼠海马神经元NeuN的表达。结果五组苏醒后即刻pH值、PaO2、PaCO2、HCO-3、BE和SaO2组间差异无统计学意义。第2天~第6天P2组、P3组和P4组大鼠逃避潜伏期随着丙泊酚剂量的增加而明显长于C组和P1组(P<0.05);与P2组比较,第2天~第6天P3组、P4组大鼠逃避潜伏期明显延长(P<0.05);与P3组比较,第2天~第6天P4组大鼠逃避潜伏期明显延长(P<0.05)。P2组、P3组和P4组大鼠穿越平台分别为(4.23±1.08)次、(2.71±1.24)次和(2.83±1.05)次,明显少于C组的(8.09±2.12)次和P1组(7.67±2.56)次(P<0.05)。与C组比较,P1组、P2组、P3组和P4组海马CA1区、CA3区和齿状回NeuN阳性神经元表达逐渐减少(P<0.05);与P1组比较,P2组、P3组和P4组海马CA1区、CA3区和齿状回NeuN阳性神经元表达逐渐减少(P<0.05);与P2组比较,P3组和P4组海马CA1区、CA3区和齿状回NeuN阳性神经元表达逐渐减少(P<0.05);与P3组比较,P4组海马CA1区、CA3区和齿状回Ne-uN阳性神经元表达逐渐减少(P<0.05)。Western blot法海马组织C组、P1组、P2组、P3组和P4组NeuN蛋白表达量分别为(1.14±0.08)>(0.85±0.06)>(0.65±0.07)>(0.52±0.07)>(0.41±0.05)(P<0.05)。结Objective To observe the long-term effects of propofol anesthesia in neonatal rats on cognitive function and hippocampal NeuN expression when growed up. Methods One hundred male sprague dawley rats aged 7 d were randomly divided into 5 groups (n=20 each) : control group (group C) and propofol groups which received intraperitoneal (IP) propo^ol 25, 50, 100 and 200 mg/ kg respectively (groups P1, P2, Pa, P4). Arterial blood samples were obtained immediately after the animals were fully recovere for blood gas analysis. The rats were feeded up until 9 weeks old and spatial learning and memory were tested by Morris water maze. The animals were then killed and their brains were removed for detection of NeuN expression in hippocampus by immunohistochemistry method and Western blot analysis. Results There was no significant difference in PaO2, PaCO2, SaOz, HCO3- ,BE and pH among the 5 groups. Compared with groups C and P1, the escape latency was longer in groups P2, P3 and P4 by dosedependent way. The frequency of passing through the platform in groups P2 ,P3 and P4 was (4.2±1.08) ,(2.71±1.24) and (2.83±1.05), which was less than that in groups C and Px. NeuN positive neurons and NeuN protein were decreased in groups P1, P2, Pa and P4 by dose-dependent way. Conclusion Propofol may have long-term effects on the development of brains in neonatal rats. Propofol may impair spatial learning and memory function of adult rats by dose-dependent down regulation of neurons and NeuN expression.
