摘  要:目的探讨血清Clara细胞分泌蛋白(CC16)、肺表面活性蛋白A(SP-A)在新生儿急性肺损伤(ALI)/急性呼吸窘迫综合征(ARDS)诊断中的作用。方法选取2011年9月至2012年12月珠海市人民医院收治的50例ALI/ARDS新生儿作为观察组,其中ALI组35例,ARDS组15例;将同期健康体检的50例新生儿作为健康对照组。采用ELISA法检测3组患儿血清CC16及sP-A水平,比较3组患儿血清CC16、SP-A水平的变化。结果1.血清CC16:ALI组[(34.19±1.09)mg/L]、ARDS组[(55.28±0.82)mg/L]明显高于健康对照组[(12.67±1.10)mg/L](P均〈0.05),且ALI组与ARDS组比较差异亦有统计学意义(P〈0.05)。2.血清SP-A:ALI组[(56.78±3.48)mg/L]、ARDS组[(70.23±5.14)mg/L]明显高于健康对照组[(22.10±3.32)mg/L](P均〈0.05),且ALI组与ARDS组比较差异也有统计学意义(P〈0.05)。结论血清CC16、sP-A可以作为早期监测新生儿ALI/ARDS的有效指标。Objective To investigate the effect of serum Clara cell secretory protein 16 ( CC16 ), pulmonary surfactant protein A(SP-A) in diagnosis of acute lung injury (ALl)/acute respiratory distress syndrome(ARDS) in newborn infants. Methods Fifty ALI/ARDS newborn infants in Zhuhai People's Hospital between 2011 Sep. and Dec. 2012 were selected as treatment group( ALl group including 35 cases, ARDS group including 15 cases),50 healthy newborns were selected as healthy control group. The changes of serum CC16, SP-A were detected by enzyme-linked im- muno sorbcnt assay and were compared in the 3 groups. Results 1. Serum CC16 in ALI group and ARDS group were (34.19 ±1.09 ) mg/L, (55.28 ± 0.82 ) mg/L, respectively, which were significantly higher than that in healthy control group [ (12.67 ± 1. 10) mg/L] (all P 〈 0.05 ), and there was significant difference between ALl group and ARDS group ( P 〈 0.05 ). 2. Serum SP-A in ALl group and ARDS group were ( 56.78 ± 3.48 ) rag/L, ( 70.23 ± 5.14) mg/L respectively,which were significantly higher than that in healthy control group [ (22. 10 ± 3.32 ) nlg/L] (all P 〈 0.05 ) , and there was significant difference between ALI group and ARDS group ( P 〈 0.05 ). Conclusion Serum CC16, SP-A can be effective parameter for early monitoring ALUARDS in newborn infants.
