我院128例肾移植术患者围术期抗菌药物应用分析

摘　　要：目的:调查了解抗菌药物专项整治活动前后我院肾移植住院患者围术期抗菌药物临床使用情况,为临床合理用药提供依据。方法:采用回顾性调查方法,分别随机抽取抗菌药物专项整治活动前(2007-2009年)、后(2011-2012年)我院器官移植中心肾移植术住院患者的88、40份病历,对肾移植围术期用药品种、用药时机、用药疗程、联合用药等情况进行统计分析。结果:抗菌药物专项整治活动前的88例肾移植患者围术期抗菌药物的使用率100%;使用频度最高的药物为注射用头孢哌酮/他唑巴坦钠、注射用头孢甲肟及甲磺酸帕珠沙星氯化钠注射液;围术期预防用药单用一种的有46例、二联用药有40例,治疗用抗菌药物二联用药有21例,三联用药有2例;术前2h内使用的只有6例,术前1d使用的有56例,术中使用的有4例,手术结束立即使用的有22例;围术期预防性应用抗菌药物最短为1d有1例,≤7d的有10例,>7～12d的有50例,12d以上的有27例;头孢呋辛钠药物利用指数值(DUI)为1.33,其余均≤1。抗菌药物专项整治活动后的40例肾移植患者围术期抗菌药物的使用率100%;使用频度最高的药物为注射用头孢哌酮钠/他唑巴坦钠、注射用头孢噻肟/舒巴坦钠及头孢呋辛酯片;围术期预防用药单用一种的有32例,二联用药有8例;术前2h内使用的有21例,术前1d使用的有5例,术中使用的有7例,手术结束立即使用的有7例;围术期预防性应用抗菌药物≤7d的有18例,>7～12d的有14例,12d以上的有8例;DUI值>1的有8个品种。结论:我院肾移植术患者围术期抗菌药物应用基本合理,但仍存在一些问题;经过抗菌药物专项整治活动,这些问题得到初步改善,主要表现在预防使用给药时机及用药疗程等方面。OBJECTIVE：To investigate the application of antibacterials in the inpatients with renal transplantation of our hospital before and after special rectification,and to provide reference for rational drug use in the clinic.METHODS：By retrospective study,88 medical records of the inpatients with renal transplantation randomly collected from our hospital before special rectification （2007-2009） and 40 medical records after special rectification （2011-2012） were analyzed statistically in terms of drug types,medication timing,duration,drug combination,etc.RESULTS：Before antibacterials special rectification,all of 88 cases were given the antibacterial drugs in perioperative period;top 3 drugs in the list of DDDs were Cefoperazone/tazobactam sodium for injection,Cefmenoxime for injection and Pazufloxacin mesilate and sodium chloride injection.Only 1 kind of drug was used in 46 cases and 2 kinds of drugs were used in 40 cases during perioperative period;2 kinds of antibacterials were used in 21 cases and 3 kinds of antibacterials were used in 2 cases.Only 6 cases were given drugs 2 hours before operation;56 cases were given drugs a day before operation;4 cases were given drugs during operation;22 cases were given drugs at the end of operation.1 case was given antibacterials for a day during perioperative period;10 cases were given antibacterials no more than 7 days;50 cases were given antibacterials for 7-12 days;27 cases were given antibacterials more than 12 days.DUI of cephalosporins sodium was 1.33,and that of others were ≤1.After antibacterials special rectification,all of 40 cases were given the antibacterials in perioperative period;top 3 drugs in the list of DDDs were Cefoperazone sodium/tazobactam sodium for injection,Cefotaxime/sulbactam sodium for injection and Cefuroxime axetil tablets.Only 1 kind of drug was used in 32 cases and 2 kinds of drugs were used in 8 cases during perioperative period;21 cases were given drugs within 2 h before operation;5 cases were given drugs a day before operation;
