The Comparison of Effect Between Double Steel Wire Internal Fixation and Absorbable Screw Fixation in Treating Patellar Fracture

摘  要:目的探讨应用可吸收螺钉及双股钢丝内固定治疗髌骨骨折的临床效果。方法 158例髌骨骨折患者,随机分为两组,采用髌前横弧形切口切开暴露髌骨骨折,手法复位后大巾钳或点式复位钳暂时固定,然后分别采用双股钢丝及可吸收螺钉加1号可吸收缝线进行内固定治疗。结果全部患者均获随访,随访时间6~15个月,平均8.5个月,双股钢丝组全部于术后一年左右二次手术取出钢丝,有22例钢丝未能完全取出。可吸收螺钉组骨折愈合时间及术后优良率均明显优于双股钢丝组,经t检验,差异有统计学意义(P〈0.05)。结论应用可吸收螺钉加1号可吸收缝线内固定治疗髌骨骨折,骨折愈合时间短,膝关节功能好,并发症少,效果优于双股钢丝固定。Objective To compare the clinical effect of absorbable screw fixation and double steel wire internal fixation in treating patellar fracture. Methods 158 cases of patellar fracture patients were randomly divided into 2 groups, absorbable screw group(n= 82)and double steel wire group (n=76). First, using anterior patellar transverse arc incision to expose the patellar fracture site, then using big towel clamp or pint reduction forceps to fix temporarily after manual reposition, and then the double steel wire group adopted double steel wire internal fixation while the absorbable screw group adopted absorbable screw plus No.1 absorbable sutures fixation treatment. Results All the patients were followed up for 6-15 months, 8.5 months on average. The double steel wire group removed the double steel wire after a year or so by operation, and 22 cases failed to completely remove the wire. The absorbable screw group healing time and postoperative quality were significantly better than that of the double wire group, by t test, the difference has statistical significance (P〈0.05). Conclusion The application of absorbable screws plus No.1 absorbable suture fixation for the treatment of patellar fracture is better than double steel wire internal fixation for its fracture healing time is shorter, postoperative quality is better and it has fewer complications.
