《中成药临床应用指导原则》实施前后我院中成药使用情况对比分析

摘　　要：目的我院临床药师根据《中成药临床应用指导原则》(下称《指导原则》)制定了《中成药临床应用指导原则实施细则》(下称《实施细则》),通过对临床数据的调查分析,观察《实施细则》的实施成效,以期发现监管中的问题,探索中成药合理应用的监管办法。方法通过回顾性分析分析住院部各科在《实施细则》发布前后的用药情况,对比前后开具中成药的病历比例、各患者住院期间使用的成药费和总药费、有无经过中医辨证而使用中成药等数据。结果在《实施细则》发布后全院使用中成药的比例、中成药费用占总药费的比例有所下降,但结果无显著性差异,西医师经过中医辨证使用中成药的情况有所增加,结果有显著差异。结论《实施细则》对临床中成药的使用率影响不大,但一定程度上提高了中成药的合理使用率,表明细则初见成效,但中成药应用走向规范化还有相当一个过程,目前急需构建中成药临床应用的指征体系和疗效评价体系。Objective To evaluate the effect of Detailed Rules of Implementation of Guidelines of Chinese Patent Medicine （ Rules I developed according to Guidelines of Chinese Patent Medicine （ Guidelines ） through a survey of clinical data and to find out the problems and the Methods of supervision in the rational use of medicine. Methods The data of proportion of Chinese patent medicine prescribed, the cost of Chinese patent medicine and all the medicine and the consideration of Bian - zheng before and after the application of Rules was analyzed retrospectively. Results After the application of Rules, the used and cost proportion of Chinese patent medicine to that of all the medicine decreased. The consideration of Bian - zheng in doctors increased. Conclusion The Rules demonstrated certain effect in supervision of Chinese patent medicine. But the standard application needed to be improved and the system of indication of application and evaluation of effect needed to be established in the future.
