摘　　要：目的评估广州市越秀区重性精神病患者实施社区综合干预实施干预后生存质量的改善效果,分析影响效果的因素。方法随机选择广州市越秀区辖区内5条行政街按诊断标准患有精神分裂症、心境障碍、分裂情感性精神病、偏执性精神障碍患者中有肇事肇祸倾向判定实行统一的暴力行为标准1～5级者145例为干预对象,进行追踪随访,在干预对象入组时、接受干预满2年时分别进行问卷调查。结果调查139例,干预前的2010年6月及干预满2年(2012年6月)时患者生存质量量表总分均值由基线时的108.41分下降到了2年后的103.22分,差异有统计学意义(P<0.01);症状、生理维度得分明显低于干预前,差异有统计学意义(P<0.01);精神病史、是否参加免费门诊和是否独居对干预前后生存质量量表总分改变值有影响。结论实施社区综合干预对改善重性精神病患者的生存质量有着积极的作用。Objective To assess the effect on the life quality by implementation of community intervention in severe psychotic,and analyze the influence factors before and after the intervention. Methods A total of 145 patients of the schizophrenia,mood disorder,schizoaffective psychosis and paranoid disorder from five streets in Yuexiu district were ran- domly selected as the object of study. They were judged violent grading standards 1-5 levels. The patients were followed up and a questionnaire survey was conducted respectively before being intervened and over 2 years. Results The 139 pa- tients were studied from June 2010 to June 2012. Compared to the previous,the score of Quality of Life Scale decreased from 108.41 to 103.22 after two years intervention,difference was statistically significant（ P 〈0.01）. The score of Syrup toms Scale and physiological Scale was significantly lower than before the intervention, difference was statistically signifi- cant（ P 〈0.01）. The influence factors were psychiatric history, whether to participate in the free psychiatric clinic, wheth er living alone. This had an effect on the Scores of Quality of Life Scale changing value. Conclusion Implementation of community intervention plays a positive role in improving the quality of life among patients with severe mental illness.
