2012年潍坊市疑似预防接种异常反应分析

摘　　要：目的分析潍坊市2012年疑似预防接种异常反应(Adverse Events Following Immunization,AEFI)的发生特征,评价AEFI监测系统运转情况及预防接种安全性。方法采用描述性流行病学方法对潍坊市2012年1月1日至12月31日报告的AEFI个案数据进行分析。结果 2012年潍坊市合计报告AEFI 1 012例,报告发生率为36.76/10万。报告发生率,一般反应为36.10/10万,异常反应为0.51/10万,偶合症为0.07/10万,心因性反应为0.04/10万;百白破(无细胞)疫苗为109.84/10万,流脑A+C结合疫苗为83.50/10万,含麻疹成分疫苗为37.36/10万,乙脑(减毒)疫苗为44.96/10万,Hib疫苗为59.06/10万。1 012例AEFI中,男性占57.61%,女性占42.39%,<1岁者占53.46%;临朐县占18.58%,寿光市占13.74%,潍城区占13.54%;5～6月占28.26%。一般反应的994例中,发热占62.47%,局部红肿占26.26%;异常反应的14例,卡介苗淋巴结炎和过敏性皮疹分别占35.71%和64.29%。全市报告县覆盖率、48h报告及时率、48h调查及时率和个案调查完整率均达到100.00%。结论全市2012年AEFI主要发生在小年龄儿童,使用疫苗不良反应报告发生率均在预期发生范围内,安全性良好。Objective To analyze the occurrence features of Adverse Events Following Immunization （AEFI） in We- ifang city, 2012, so as to evaluate the implementation of AEFI surveillance system and the immunization safety of vaccines. Methods Data were collected through the AEFI Information System,which reported from January 1 to December 31, 2012. The descriptive methodology was used. Results A total of 1 012 AEFI cases were reported and the report inci dence was 36.76/10^5. The report incidence of AEFI cases were different, the general reaction was 36.10/10^5 , the paradox- ical reaction was 0.51/10^5 ,the coupling system was 0.07/10^5 ,and the psychogenic reaction was 0.04/10s ;and the DTaP Vaceine was 109.84 / 10^5 , the Meningococcal Vaccine eonjugating A and C group was 83.50/10^5 thousand, the Vaccine of containing Measles and Parotitis was 37.36/10^5 ,JEV-Live was 44.96/10^5 one hundred thousand and the Hib Vaccine was 59.06/10^5. Among 1 012 AEFI cases,male and female respectively accounted for 57.61~/00 and 42.39%. Targets of AEFI cases aged ≤ year accounted for 53.46% ;The reporting cases were different in Linqu eounty,Shouguang county and Weicheng district,respectively accounted for 18.58%, 13.74%, and 13.54% ;The reporting cases accounted for 28. 26% in May and June. Among 994 cases of general reaction, fervescence and local swelling respectively accounted for 62.47 % and 26. 260%; 14 cases of paradoxical reaction, Anaphylactic Rash and BCG lymphadenitis respectively accounted for 35.71% and 64.29 %. The coverage rate of reporting counties, the reported rate within 48 h, the investigated rate within 48 h and intaet rate of cases were 100.00% ih whole city. Conclusion AEFIs often occur in young children,the esti- mated reporting rate of vaccine reactions is not over the expected, the vaccines safetv is well.
