摘 要:目的探讨Kiss-1、β-catenin蛋白在甲状腺乳头状癌(PTC)中的表达与颈部淋巴结转移的关系。方法采用免疫组织化学方法检测86例PTC组织和15例正常甲状腺组织中Kiss-1、β-catenin蛋白的表达。结果①正常甲状腺组织的Kiss-1阳性表达率显著高于PTC(χ2=14.16,P<0.01);无伴有颈部淋巴结转移的PTC的Kiss-1阳性表达率显著高于伴有淋巴结转移者(χ2=15.41,P<0.01);微小癌与非微小癌两者的Kiss-1阳性表达率间的差异不具有统计学意义(χ2=3.47,P>0.05)。②PTC的β-catenin异常表达率显著高于正常甲状腺组织(χ2=24.75,P<0.01);微小癌和非微小癌、伴有和无伴有颈部淋巴结转移的PTC间的β-catenin异常表达率的差别均无统计学意义(χ2值分别为0.35和0.53,P>0.05)。③Kiss-1阴性表达与β-catenin异常表达正相关(spear-man correlation value=0.56,P<0.01)。结论β-catenin的异位表达很可能始于PTC发生发展过程中的早期阶段,Kiss-1表达的下调可能级联增强了β-catenin的正向效应作用,最终导致PTC的发生和颈部淋巴结的癌转移。Objective To explore the expressions of Kiss-1, β-catenin protein in papillary thyroid carcinoma (PTC) and their relations to cervical lymph node metastases. Methods The expressions of Kiss-1, β-catenin protein were detected in 86 cases of PTC and 15 cases of normal thyroid tissue by immunohistochemistry. Results (~)The positive expression rate of Kiss-1 in normal thyroid tissue was significantly higher than that in PTC (χ2= 14.16,P 〈 0.01 ). The positive expression rate of Kiss-1 in PTC without cervical lymph node metastases was significantly higher than that in PTC with cervical lymph node metastases (χ2=15.41 ,P 〈 0.01 ). The difference of the positive expression of Kiss-1 between microcarcinoma and non-microcarcinoma was not statistically significant (χ2=3.47 ,P 〉 0.05). ②The abnormal expression rate of β-catenin in PTC was significantly higher than that in normal thyroid tissue. The difference of the abnormal expression rate of β-catenin either between microcarcinoma and non-microcarcinoma, or between PTC with cervical lymph node metastases and without cervical lymph node metastases was not statistically significant (χ2 value 0.35, 0.53 respectively, P 〉 0.05). ③There was a positive correlation between the negative expression of Kiss-1 and the abnormal β-catenin expression in PTC. Conclusion The abnormal expression of β-catenin protein may be the early event during the tumorigenesis and development of PTC. The down-regulation of the expression of Kiss-1 protein may have cascade amplification on the positive effect of β-catenin protein, which finally leads to cervical lymph node metastases of PTC.
