大便镜检中形态相似和少见寄生虫的形态特征及鉴别

摘　　要：目的提高临床检验人员大便常规检测中发现和辨认寄生虫的能力。方法通过对2006年～至今7年来参加美国病理家学会寄生虫能力比对试验85份标本和资料的分析,总结大便镜检中形态相似和少见寄生虫的的形态特征。结果形态相似寄生虫卵鉴别诊断包括:裂头绦虫卵、肺吸虫卵、姜片虫卵和肝片虫卵;少见和输入性寄生虫的鉴别包括:埃及血吸虫卵、曼氏血吸虫卵和贝氏等胞球虫包囊;粪便中食物残渣和虫卵鉴别包括:粪类圆线虫幼虫和纤维丝,肝吸虫卵和灵芝孢子,绦虫卵、蛔虫卵和花粉孢子;包囊和细胞的鉴别包括:各类包囊和白细胞,人芽囊原虫和红细胞。结论定期进行寄生虫图谱复习、混合虫卵样本的镜下复习、特殊寄生虫和虫卵的复核等,可提高大便常规中寄生虫检验质量,提升检验人员能力。Objective To enhance the technicians＇ ability to detect and identify the parasites in stool samples.Methods The experiences about how to detect and identify the parasites in the stool samples were drawn by analyzing the data of 85 samples from CAP proficiency test of parasitology from 2006 to 2012.Results Differential diagnosis of eggs in similar shape including： egg of Diphyllobothriun latum,egg of Paragonimus,egg of Fasciolopsis buski and egg of Fasciola hepatica;differential diagnosis of rare and imported parasites including： egg of Schistosoma haematobium and egg of S.mansoni,Isospora belli;differential diagnosis of food debris and eggs including： larvae of Strongyloides stercoralis and fiber,egg of Clonorchis sinensis and ganoderma spores,egg of Taenia,egg of Ascaris lumbricoides and pollen spores;differential diagnosis of cysts and cells including： cysts of protozoa and white blood cell,Blastocystis hominis and red blood cell.Conclusion The quality of parasites examination in stool samples is improved and the technicians＇ ability is also enhanced.
