摘  要:增强型体外反搏(EECP)是一种非侵入性、成本低,且安全有效的冠状动脉粥样硬化性心脏病(冠心病)治疗措施,然而其确切的疗效机制至今仍未得到充分的阐明。以往认为,血流动力学效应是EECP发挥作用的主要机制。近年研究表明,EECP增加血流切应力,并通过血管生物学机制发挥血管保护和抗动脉粥样硬化进展的益处。EECP诱导的血流切应力的增加可改善血管内皮的功能与形态,减轻机体氧化应激和炎症反应,以及促进血管新生和血管形成。本综述将复习EECP发挥即时和长期效益的潜在机制和相关证据,并从其增加血流切应力的角度着重论述。Enhanced external counterpulsation(EECP) is a non-invasive therapy for patients with coronary artery disease,which is proven to be safe,low-cost,and beneficial.The mechanisms of action for EECP,however,have not been fully elucidated.Previously,the hemodynamic effects of EECP were thought to be its major mechanism of action.Recent advances have shown that EECP increases endothelial shear stress,which results in vascular protection and alleviation of atherosclerosis.EECP-induced shear stress increase may lead to improvement in endothelial function and morphology,attenuation of oxidative stress and inflammation,and promotion of angiogenesis and vasculogenesis.This article reviews relevant evidence of EECP with regards of its shear stress-increasing effects.
