2010年与2011年某院临床常见病原菌及耐药菌谱分析  

Distribution and drug resistance analysis of common pathogenic bacterium in Beijing Traditional Chinese Medical Hospital from 2010 to 2011

摘  要:目的对该院2010年与2011年临床常见病原菌及耐药菌谱进行比较分析,为临床合理用药提供依据。方法该院2010-2011年度住院患者送检的细菌培养标本,所有培养出细菌的标本均进行细菌鉴定,致病菌标本同时进行药敏试验分析。药物敏感性试验和结果判断标准按CLSI2010年版规定,数据处理用wHONET5.4软件进行分析。结果2010、2011年全年病房共送检细菌培养标本分别为6237份和6600份,其中致病菌分别为2046株和2136株,阳性率分别为32.8%、32.4%。2010、2011年全年病房送检的阳性标本中革兰阳性菌分别占40.7%、24.9%,以金黄色葡萄球菌、表皮葡萄球菌、粪肠球菌、屎肠球菌多见;革兰阴性菌分别为59.3%、75.1%,以大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌、鲍曼不动杆菌、阴沟肠杆菌多见。其中耐苯唑西林金黄色葡萄球菌(MRSA)分剐占金黄色葡萄球菌总数的12.2%、11.4%;耐万古霉素肠球菌(VRE)分剐占肠球菌总数的15.5%、14.5%;泛耐药鲍曼不动杆菌分别占鲍曼不动杆菌的39%、21.6%;产ESBLs细菌分别占革兰阴性杆菌的21.1%、17.6%。结论加强对抗菌药物的管理,合理选择抗菌药物种类,减少细菌耐药性的发生,进一步提高临床抗感染治疗的效果。Objective To compare the distribution and drug resistance of common pathogenic bacterium between 2010 and 2011and provide basis for rational clinical administration. Methods Bacteria were isolated and identified routinely,and antimicrobial susceptibility test was performed by using Vitek 2 automated microbiological assay system. Procedure and results criteria of drug susceptibility test referred to 2010 edition standards of Clinic and Laboratory Standard Institute and data were analyzed by using WHONET 5.4 software. Results In 2010 and 2011,2046 (32.8%) and 2136 (32.4o//00) strains of pathogenic bacterium were isolated from 6237 and 6600 specimens. 40.7~ and 24. 9o//oo of pathogenic bacterium were Gram-positive bacteria, majority of which were Staphylococcus aureus, Staphylococcus epidermidis,Enterococcus faecalis and Eenterococcus faecium, and Gram-negative bacteria accounted for 59.30//oo and 75. l~//0, majority of which were Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, Acinetobacter baumannii and Enterobacter cloacae. In 2010 and 2011,the percentages of methicillin resistant Staphy- lococcus aureus (MRSA) in all Staphylococcus aureus strains were 12.2% and 11.4% ,those of vancomycin resistant enterococcus (VRE) in all enterococci strains were 15.5 ~ and 14. 5 ~/0, those of pan-drug resistant Acinetobacter baumannii (PDR-AB) in all Acinetobacter baumannii strains were 39% and 21.6% ,and those of bacteria producing ESBLs in all Gram-negative bacilli were 21.1 % and 17.6 %. Conclusion The management of antibacterials should be strengthened and antibiotics should be rationally used to reduce the occurrence of drug resistance and further improve the clinical effects of anti-infective therapy.
