摘  要:目的监测社区感染大肠埃希菌的耐药性及其变迁,为合理用药提供科学依据。方法使用VITEK-2对2010-2011年符合社区感染患者的各种标本580株进行常规药敏试验,并比较其两年的耐药性及其变迁。结果两年收集到社区感染大肠埃希菌分别为259株、321株,产ESBLs菌株检出率分别为50.6%、51.4%。药敏结果显示580株大肠埃希菌对厄他培南、亚胺培南耐药率低于1.0%,对哌拉西林/他唑巴坦的耐药率低于0.3%,产ESBLs菌株对庆大霉素、环丙沙星、左旋氧氟沙星的耐药率均高于45.0%。2011年出现了对碳青霉烯类抗生素耐药的菌株。结论 2010年与2011年社区感染大肠埃希菌对常用抗菌药物的耐药率差异无统计学意义(P>0.05)。加强抗菌药物管理,合理使用抗菌药物是减少耐药株产生的关键。Objective To study the clinical distribution and antimicrobial resistance ofEscherichia coli, from community acquired infections, for the guidance of rational use of antibiotics. Methods The identification and the drug susceptibility test were analyzed by VITEK-2 automatic microbiology analytical system. The clinical distribution and anfimicrobial resistance of E. coli were compared from 2010 to 2011. Results The number of E. coli strains from community acquired was 259 in 2010, 321 in 2011. The rate of ESBLs (+) strains in 2010 was 50.6% and 51.4% in 2011. The drug susceptibility test of the 580 strains showed that the resistance rates to Ertapenem and Imipenem were lower than 1.0%. The resistance rate to Piperacillin/Tazobactam was lower than 0.3%. The ESBLs (+) strains' drug resistance rates to Gentamicin, Ciprofloxacin and Levofloxacin were higher than 45.0%, The Carbapenem-resis- tant strains were observed in 2011. Conclusion There is no statistically significant difference in the antimicrobial re- sistance ofE. coli to commonly used drugs from 2010 to 2011 (P〉0.05). In order to reduce the drug resistance rate, it's necessary to use antibiotics rationally.
