摘 要:目的探讨乳汁成分对乙型肝炎病毒DNA(HBV-DNA)检测的影响以及乙肝病毒DNA在乳汁中的分布情况,为建立可行、标准的乳汁HBVDNA检测方法提供依据。方法建立乳汁成分干扰模型、生理盐水对照模型,比较干扰模型与对照模型中的HBV-DNA检测结果。在HBV-DNA阴性乳汁中掺入高中低值标准HBVDNA阳性血清,分别取样本乳酪层、乳清与沉淀进行HBV-DNA检测。结果乳酪、乳清、沉淀干扰模型分别与盐水对照模型比较差异均有统计学意义(t值分别为3.30、3.00、4.60,P<0.05)。乳酪、乳清、沉淀干扰模型间相互比较,差异均无统计学意义(P>0.05)。在掺入高值和中值标准血清时,HBV-DNA在乳酪、乳清和沉淀中的测定结果两两比较差异均有统计学意义(P<0.05),其中沉淀中的测定结果最高,其次为乳清,最低的为乳酪层;在掺入低值标准血清后,除乳酪与沉淀中的HBV-DNA测定结果差异有统计学意义(P<0.05)外,其余差异均无统计学意义(P>0.05)。结论乳汁主要成分对于乳汁样本中HBV-DNA的检测具有干扰作用,使得测定值低于实际值。乳汁样本中HBV-DNA主要存在于沉淀部分,乳清、乳酪中也同时存在HBV-DNA。Objective To investigate the influence of the compositions of human breast milk on the quantitative detection of HBV-DNA and the distribution of HBV-DNA in the human breast milk,and to provide the basis to set up a feasible and standard method for the quantitative detection of HBV-DNA in the human breast milk.Methods We set up the models interfere with cheese,whey,sediment and the compared model of physiological saline,and contrasted the HBV-DNA quantitative detection of interferential-model with compared-model.Then we mixed the high-value,mid-value and low-value standard HBV-DNA positive serum with HBV-DNA negative breast milk,and detected the HBV-DNA load in the cheese,whey and sediment.Results Contrasted the HBV-DNA quantitative detection of the cheese interferential-model,whey interferential-model,sediment interferential-model with the physiological saline compared-model,the value of t were 3.30,3.00 and 4.60,and the value of P were less than 0.05.Contrasted the HBV-DNA quantitative detection between the cheese interferential-model,whey interferential-model and sediment interferential-model,the value of P were greater than 0.05.In the condition of mixed negative breast milk with high-value and mid-value standard serum,contrasted the HBV-DNA quantitative detection between the cheese,whey and sediment,the value of P were less than 0.05,and the quantity of HBV-DNA in the sediment was highest,in the whey was secondly and in the cheese was lowest.In the condition of mixed negative breast milk with low-value standard serum,the value of P were greater than 0.05 except the value of P contrasted between cheese and sediment was less than 0.05.Conclusion The compositions of human breast milk can interfere the quantitative detect of HBV-DNA,which makes the estimated value less than the actual value.The HBV-DNA mostly exists in the sediment,also in the cheese and whey.
